甲状腺功能亢进症患者血皮质醇、促肾上腺皮质激素含量水平与疾病程度相关性研究

摘　　要：目的:分析甲状腺功能亢进症(甲亢)患者血皮质醇、促肾上腺皮质激素(ACTH)水平与病情严重程度的相关性。方法:回顾性分析80例甲亢患者(甲亢组)和35例同期体检的健康人(健康组)的临床资料,比较两组受试者实验室相关指标、血皮质醇、ACTH含量水平。按病情严重程度将甲亢组患者分为轻症组、中症组和重症组,比较三亚组患者皮质醇、ACTH水平,并分析甲亢病情严重程度与皮质醇、ACTH水平的相关性。结果:甲亢组患者皮质醇、ACTH水平低于健康组,差异有统计学意义(P<0.05)。重症组患者血皮质醇、ACTH水平低于中症组与轻症组,中症组亦低于轻症组,差异有统计学意义(P<0.05)。血皮质醇、ACTH是甲亢病情严重程度的影响因素,且与甲亢病情严重程度呈负相关(r=-0.420、-0.462,均P<0.05)。结论:甲亢患者血皮质醇、ACTH水平降低,与甲亢严重程度呈负相关,在甲亢疾病严重程度评估中有一定积极作用。Objective:To analyze the correlation between serum cortisol,adrenocorticotropic hormone(ACTH)levels and disease severity in hyperthyroidism patients.Methods:The clinical data of 80 hyperthyroidism patients(hyperthyroidism group)and 30 healthy people(healthy group)who underwent physical examination during the same period were retrospectively analyzed.The laboratory-related indexes,serum cortisol and ACTH levels were compared between the two groups.According to the disease severity,the patients in the hyperthyroidism group were divided into mild group,moderate group and severe group.The levels of serum cortisol and ACTH were compared among the three subgroups.The correlation between the degree of hyperthyroidism and the levels of serum cortisol and ACTH was analyzed.Results:The levels of serum cortisol and ACTH in the hyperthyroidism group were lower than those in the healthy group(P<0.05).The levels of serum cortisol and ACTH in the severe group were lower than those in the moderate group and the mild group,which were lower in the moderate group than that in the mild group(P<0.05).Serum cortisol and ACTH were the influencing factors of hyperthyroidism severity,which were negatively correlated with hyperthyroidism severity(r=-0.420,-0.46,all P<0.05).Conclusion:The levels of serum cortisol and ACTH in patients with hyperthyroidism are decreased,which have a negative correlation with thyperthyroidism severity,and has a positive effect on the evaluation of hyperthyroidism severity.
