尿液免疫球蛋白轻链检测对系统性红斑狼疮肾脏损伤的诊断价值

摘　　要：目的:探讨人体尿液中免疫球蛋白轻链kappa型(U-kap)和lambda型(U-lam)在系统性红斑狼疮(SLE)患者肾损伤中的诊断价值。方法:采用回顾性研究方法,收集系统性红斑狼疮50例作为SLE组。另收集健康体检者20例作为对照组。比较两组尿液中U-kap、U-lam水平,分析U-kap、U-lam在SLE患者肾脏损伤中的诊断价值。结果:SLE组和对照组U-kap、U-lam水平比较,差异均有统计学意义(P<0.05)。ROC曲线分析显示,U-kap=6.94 mg/L对肾损伤诊断预测的敏感度为94%,特异度为100%,曲线下面积(AUC)为0.951;U-lam=3.81 mg/L对肾损伤诊断预测的敏感度为94%,特异度100%,AUC为0.967。结论:尿液中U-kap、U-lam在SLE患者肾脏损伤中具有良好的临床诊断价值。Objective:To investigate the diagnostic value of immunoglobulin light chain kappa(U-kap)and lambda(U-lam)in the urine of patients with systemic lupus erythematosus(SLE).Methods:A retrospective study was conducted to collect 50 SLE patients as SLE group.Another 20 healthy people were collected as control group.The levels of U-kap and U-lam in urine samples of the two groups were compared,and the diagnostic value of U-kap and U-lam in renal injury of SLE patients was analyzed.Results:There was significant difference between SLE group and control group in the levels of U-kap and U-lam(P<0.05).ROC curve analysis showed that the sensitivity,specificity and AUC of U-kap=6.94 mg/L were 94%,100%and 0.951 respectively,and the sensitivity,specificity and AUC of U-lam=3.81 mg/L were 94%,100%and 0.967 respectively.Conclusion:U-kap and U-lam in urine have a good clinical diagnostic value in SLE patients with renal injury.
