多轴锁定接骨板与前外侧L形锁定加压接骨板内固定治疗较短胫骨远端干骺端骨折的疗效比较

摘　　要：目的对比经皮微创多轴锁定接骨板固定与前外侧"L"型锁定加压接骨板内固定治疗较短胫骨远端干骺端骨折的临床疗效。方法回顾性分析2015-01/2017-01月在作者医院治疗的35例较短胫骨远端干骺端骨折患者临床资料(远断端距关节面距离<4.2cm),其中20例采用"L"型锁定加压接骨板内固定(L型锁定组),15例采用经皮微创内固定技术(minimally invasive percutaneous osteosynthesis,MIPO)接骨板固定(多轴锁定组)。记录并比较两组的手术时间、术中出血量、骨折愈合所需时间、并发症发生情况及临床疗效等。结果对所有患者进行随访,平均随访时间为15.5个月。多轴锁定组平均手术时间、术中出血量、骨折愈合所需时间均明显少于L型锁定组(P均<0.05);而多轴锁定组并发症发生率和临床治疗效果与L型锁定组相比,无统计学差异(P>0.05)。结论经皮微创多轴锁定接骨板固定在较短胫骨远端干骺端骨折治疗中,疗效较佳,可缩短手术时间和骨折愈合所需时间,减少术中出血量。Objective To compare the effects of multi-axial locking plates and anterior lateral L-shaped locking compression plate internal fixation for distal metaphyseal fractures of tibia.Methods The clinical data of 35 patients with short distal metaphyseal fracture of tibia(distance from distal end to articular surface<4.2 cm)treated in authors′hospital from January2015 to January2017 were retrospectively analyzed.Of which20 patients were treated with anterior lateral L-shaped locking compression plate internal fixation(L-shape locking group),15 cases were treated with minimally invasive percutaneous osteosynthesis(MIPO)plate fixation(multi-axis locking group).Operative time,intraoperative blood loss,fracture healing time,complications and clinical efficacy of the two groups were recorded and compared.Results All patients were followed up for average of 15.5 months.The average operative time,intraoperative blood loss and fracture healing time of the multi-axis locking group were significantly shorter than those of the L-shape locking group(all P<0.05).There were no significant differences in the excellent result rate and complications rate between the two groups(P>0.05).Conclusion MIPO multi-axial locking plate fixation for distal metaphyseal fractures of tibia is effective,can shorten the time of operation and fracture healing,and reduce intraoperative blood loss.
