摘 要:目的探讨银杏叶提取物通过调控缝隙连接蛋白(Cx43)分布及活性减少大鼠心肌再灌注损伤的实验研究。方法选取健康雄性SD大鼠40只,按照体重随机分为假手术组、缺血/再灌注组、金纳多组、银杏内酯组,每组10只,建立心肌再灌注损伤大鼠模型,每组分别经对应药物处理后留取标本,并经HE染色观察大鼠心肌组织形态变化,检测血清超氧化物歧化酶(SOD)活性、氧化应激产物丙二醛(MDA)浓度以及Cx43分布、表达及灰度值(OD);记录再灌注30 min内每组室性期前收缩、室性心动过速、室颤数等心律失常临床表现次数、持续时间,经评分标准比较各组评分情况。结果缺血/再灌注组SOD活性、MDA含量均低于假手术组(均P<0.05);金纳多组SOD活性、MDA含量均高于缺血/再灌注组(均P<0.05),但银杏内酯组SOD活性、MDA含量与缺血/再灌注组比较,差异均无统计学意义(均P>0.05);缺血/再灌注组Cx43OD值与假手术组比较明显降低(P<0.05),呈现降低、紊乱;金纳多组Cx43OD值与缺血/再灌注组比较明显升高(P<0.05),表现在断端连接处分布较多,侧移降低;银杏内酯组Cx43OD值与缺血/再灌注组比较差异无统计学意义(P>0.05),表现无明显变化;缺血/再灌注组各心律失常出现次数与持续时间均高于假手术组,且心律失常的评分明显高于假手术组(均P<0.05);金纳多组室早等心律失常发生次数及持续时间均明显低于缺血/再灌注组,且心律失常评分低于缺血/再灌注组(均P<0.05),金纳多组室早等心律失常发生次数及持续时间均明显低于银杏内酯组,且心律失常评分低于银杏内酯组(均P<0.05),银杏内酯组室早等心律失常发生次数及持续时间与缺血/再灌注组比较,差异均无统计学意义(均P>0.05)。结论银杏叶提取物可调控大鼠心肌缺血后再灌注损伤,且可提升Cx43表达水平,使Cx43分布异常下降,缓解缺血再灌注损伤性心律失常。Objective To investigate the experimental study of Ginkgo biloba extract on myocardial reperfusion injury in rat by regulating the connexin 43(Cx43)distribution and activity.Methods Forty male Sprague-Dawley rats were randomly divided into sham operation group,ischemia/reperfusion group,ginaton group and ginkgolides group,10 rats in each group with corresponding drugs treatment.Myocardial reperfusion injury rat model were established.The specimen was collected and stained with HE for myocardial tissue morphology observation.The serum superoxide dismutase(SOD)activity and the oxidative stress product malondialdehyde(MDA)concentration and Cx43 distribution,expression and gray value(OD)were detected.The frequency of clinical manifestations and duration of ventricular premature contraction,ventricular tachycardia,ventricular fibrillation,within 30 min after reperfusion were recorded to compare the scores of each group by scoring criteria.Results The SOD activity and MDA content in the ischemia/reperfusion group were lower than those in the sham operation group(P<0.05);the SOD activity and MDA content in the ginaton group were higher than those in the ischemia/reperfusion group(P<0.05),while there was no significant difference in SOD activity,MDA content between ginkgolide group and ischemia/reperfusion group(P>0.05).The Cx43 OD value in the ischemia/reperfusion group was significantly lower than that in the sham operation(P<0.05),and was reduced and disordered;the Cx43 OD value of the Ginaton group was significantly higherthan that of the ischemia/reperfusion group(P<0.05),and more at the terminal junction and decreased lateral shift.There was no significant difference in the Cx43 OD value between the ginkgolide group and ischemia/reperfusion group(P>0.05)with no significant change in performance.The number and duration of arrhythmia in the ischemia/reperfusion group were higher than those in the sham operation group,and the arrhythmia score was significantly higher than that in the sham operation group(P<0.05);the n
