摘 要:目的探索雌二醇(E2)和卵泡大小的单卵泡日平均增长情况对妊娠率的影响。方法选用59例接受体外受精-胚胎移植(IVF-ET)的患者。纳入标准:原发不孕,年龄≤38岁,因输卵管因素助孕,本次为第1周期助孕,且移植第3天新鲜胚胎,均采用卵泡期长方案控制性促排卵,未发生卵巢过度刺激综合征。排除标准:生殖系统畸形,合并代谢性疾病,有子宫内膜异位症、多囊卵巢综合征、宫腔黏连等疾病。按妊娠与否分为妊娠和未妊娠2组,比较2组间血清基础性激素、抗苗勒管激素(AMH),Gn启动日血清促卵泡激素(FSH)、促黄体生成素(LH)、雌二醇(E2),最大卵泡生长至14 mm、14 mm至人绒毛膜促性腺激素(HCG)日LH、E 2,14 mm前、14 mm至HCG日E 2和卵泡大小平均日增长、14 mm前、14 mm至HCG日E 2和卵泡大小单个卵泡平均日增长的差异性。结果E 2浓度和卵泡大小的妊娠组14 mm前日增长和14 mm至HCG日日增长均低于未妊娠组,差异有统计学意义(P<0.05),而卵泡大小的总体日增长差异无统计学意义(P>0.05),E 2浓度的单个卵泡增长中妊娠组14 mm前日增长、14 mm至HCG日日增长和总体增长均低于未妊娠组,差异有统计学意义(P<0.05),卵泡大小的单个卵泡增长中妊娠组14 mm至HCG日日增长和总体增长均低于未妊娠组,差异有统计学意义(P<0.05)。结论在控制性促排卵过程中,密切监测和控制E 2和卵泡大小的单卵泡日平均增长可以提高IVF-ET妊娠率。Objective To explore the influence of the periodic single follicle size and the daily average increase value of estradiol on pregnancy rate.Methods 59 cases with assisted pregnancy were collected.The inclusion criteria:primary infertility,Ages≤38,assisted pregnancy with fallopian tube factors,the first cycle of assisted pregnancy and the fresh embryo in the third day of transplantation,long follicle period program was used to control ovulation and no ovarian hyperstimulation syndrome occurred;The patients were divided into the pregnant group and non-pregnant group.The differences of basic FSH,LH,E 2,AMH and on the start-up day of GN,maximum follicle size of 14mm and LH,E 2 on the hCG day,average daily growth of E 2 and follicle size before 14 mm,14 mm to hCG day,average daily growth of E 2 and follicle size of single follicle before 14 mm,14 mm to HCG day were compared.Results The E 2 concentration and follicle size of 14mm,14mm to HCG day of pregnant group were significant lower than those in non-pregnant group(P<0.05).However,there was no significant difference in the overall daily growth of follicle size(P>0.05).In the growth of single follicle of E 2 concentration,the daily growth before 14 mm,the daily growth from 14 mm to HCG and the overall growth of pregnancy group were lower than those in non-pregnant group(P<0.05),and the daily growth and the overall growth of HCG in the pregnant group were lower than those in the non-pregnant group(P<0.05).Conclusion In the process of controlled ovulation,closely monitor and control E 2 and follicle size can improve the rate of assisted pregnancy.
