胸腔镜下先天性食道闭锁纠治术对先天性食道闭锁患儿血清WBC、CRP水平及并发症的影响

摘　　要：目的探讨胸腔镜下先天性食道闭锁纠治术对先天性食道闭锁患儿血清白细胞计数(WBC)、C反应蛋白(CRP)水平及并发症的影响。方法选取贵阳市妇幼保健院2016年1月至2019年12月收治的40例先天性食道闭锁患儿,采用随机数表法分为对照组和观察组,各20例。对照组行食管气管瘘修补术治疗,观察组行胸腔镜下先天性食道闭锁纠治术治疗,观察两组手术时间、术中出血量及术后并发症发生率;比较术前及术后3 d两组排痰量,术前及术后1周两组血清WBC、CRP水平。结果与对照组比,观察组手术时间缩短,术中出血量减少(P<0.05);与术前比,术后3 d两组排痰量均减少,且观察组少于对照组(P<0.05);与术前比,术后1周两组血清WBC、CRP水平均降低,且观察组低于对照组(P<0.05);观察组并发症发生率为5.00%,低于对照组的35.00%(P<0.05)。结论胸腔镜下先天性食道闭锁纠治术治疗先天性食道闭锁可有效改善患儿咳痰症状,缩短手术时间,减少术中出血量,调节血清WBC、CRP水平,缓解机体炎症反应,降低并发症发生率。Objective To investigate the effect of thoracoscopic correction of congenital esophageal atresia on serum WBC and CRP levels and complications in children with congenital esophageal atresia.Methods Totally 40 children with congenital esophageal atresia treated in our hospital from January 2016 to December 2019 were randomLy divided into control group and observation group,each with 20 cases.Control group was treated with esophageal and tracheal fistula repair.Observation group was treated with thoracoscopy for congenital esophageal atresia.Observed the operation time,intraoperative blood loss and the incidence of postoperative complications in the two groups;compared the amount of sputum excretion between the two groups before surgery and 3 days after surgery,and the serum WBC and CRP levels of the two groups before surgery and 1 week after surgery.Results Compared with the control group,the operation time of the observation group shortened and the amount of bleeding during the operation reduced(P<0.05).Compared with before surgery,the sputum output of both groups decreased 3 days after surgery,and the observation group was less than the control group(P<0.05);compared with before surgery,the levels of serum WBC and CRP in the two groups decreased 1 week after surgery,and the observation group was lower than the control group(P<0.05);the incidence of complications in the observation group was 10.00%,which was lower than 35.00%in the control group(P<0.05).Conclusion Treatment of congenital esophageal atresia with thoracoscopy can effectively improve clinical symptoms such as sputum sputum,shorten the operation time,decrease the amount of intraoperative blood loss,adjust the serum WBC and CRP levels,relieve the body's inflammatory response,and reduce concurrency disease incidence.
