2型糖尿病合并小细胞性贫血病人糖化血红蛋白与空腹血糖的相关性分析

摘　　要：目的:探讨糖化血红蛋白(HbA1c)检测在2型糖尿病(DM)合并小细胞性贫血诊断中的应用价值。方法:对34例合并小细胞性贫血的2型DM病人(小细胞贫血组)、34例正细胞性贫血的2型DM病人(正细胞贫血组)、48例非贫血2型DM病人(单纯2型DM组)及68名健康体检者(健康对照组),分别检测HbA1c、空腹血糖(FPG)、血红蛋白(Hb)和平均红细胞体积(MCV)水平,分析HbA1c与FPG的相关性。结果:4组HbA1c、FPG、Hb、MCV检测结果比较,差异均有统计学意义(P<0.01)。正细胞贫血组和单纯2型DM组HbA1c与FPG存在正相关(r=0.708、0.773,P<0.05),小细胞贫血组HbA1c与FPG无相关性(r=0.302,P>0.05)。结论:2型DM合并小细胞性贫血病人HbA1c水平不能准确反映病人体内血糖真实水平。Objective:To explore the value of glycosylated hemoglobin(HbA1c)level in the diagnosis of type 2 diabetes mellitus complicated with microcytic anemia.Methods:The levels of HbA1c,FPG,Hb and MCV in 34 patients with type 2 diabetes mellitus complicated with microcytic anemia(microcytic anemia group),34 patients with type 2 diabetes mellitus complicated with normocytic anemia(normocytic anemia group),48 patients with type 2 diabetes mellitus complicated without anemia(simple type 2 DM group)and 68 healthy people(healthy control group)were detected.The correlation between HbA1c and FPG was analyzed.Results:The differences of the levels of HbA1c,FPG,Hb and MCV among four groups were statistically significant(P<0.01).The HbA1c level was positive correlation with FPG in normocytic anemia group and single type 2 DM group(r=0.708,r=0.773,P<0.05),but the HbA1c level was not correlation with FPG in microcytic anemia group(r=0.302,P>0.05).Conclusions:The HbA1c level in type 2 diabetes mellitus patients with microcytic anemia can not accurately reflect the real level of blood glucose.
