慢性阻塞性肺疾病病人社区卫生服务需求的临床现状及影响因素分析

摘　　要：[目的]调查了解慢性阻塞性肺疾病(COPD)病人社区卫生服务需求的临床现状,并分析其影响因素,为病人的社区护理提供参考依据。[方法]2018年11月—2018年12月采用便利抽样法选择本市3个社区的201例COPD病人,应用自行设计的一般资料调查表和COPD病人社区卫生服务需求调查问卷实施调查。[结果]COPD病人对社区卫生服务的需求得分为(100.95±15.23)分,在心理社会需求和健康相关行为需求的得分率较高,分别为65.5%和62.5%;通过单因素分析和多元线性回归分析可知,年龄、文化程度、独居情况、活动能力、急性发作次数和合并症情况均为影响COPD病人社区卫生服务需求的因素(F=55.542,P<0.05)。[结论]COPD病人对社区卫生服务的需求水平相对较高,并且主要体现在心理社会需求和健康相关行为需求两方面,临床医护人员应根据病人的个体特点和需求情况实施有针对性的延续性护理,以提升病人的社区康复水平。Objective:To investigate and understand the clinical status of community health service demand of patients with chronic obstructive pulmonary disease(COPD),and analyze its influencing factors,so as to provide reference basis for the community care of patients.Methods:From November to December 2018,a total of 201 patients with COPD in three communities of a city were selected by convenience sampling.The self-designed general information questionnaire and the community health service demand questionnaire of patients with COPD were used to carry out the survey.Results:The demand score of COPD patients for community health services was(100.95±15.23).The score rates of psychosocial demand and health-related behavioral demand were 65.5%and 62.5%respectively.Through univariate analysis and multiple linear regression analysis,it was found that the age,education level,solitude,activity ability,number of acute episodes and complications were the influencing factors of community health services demand of patients with COPD(F=55.542,P<0.05).Conclusions:COPD patients had a relatively high level of demand for community health services,and mainly embodied in psychosocial needs and health-related behavioral needs.Clinicians and nurses should implement targeted continuing care according to the individual characteristics and needs of patients,in order to improve the level of community rehabilitation of patients.
