摘 要:目的:评估自制胸腰椎体表定位器在脊柱微创手术中应用效果。方法:选取2017年1月~2018年12月收治的139例胸腰椎疾患拟行射频消融术及椎体成形术治疗患者,随机分为观察组(射频消融27例34个节段,椎体成形42例49个椎体)和对照组(射频消融术27例35个节段,椎体成形术43例45个椎体)。观察组采用胸腰椎体表定位器定位,对照组采用传统克氏针定位,统计两组使用不同定位方法的透视、穿刺次数,并进行比较分析。结果:在定位、穿刺透视次数,观察组中射频消融术定位透视平均(1.86±0.94)次、穿刺平均透视(4.52±2.01)次;椎体成形术定位平均透视(2.15±1.34)次、穿刺平均透视(3.12±2.16)次。对照组中射频消融术定位平均透视(3.47±1.28)次、穿刺平均透视(8.75±3.37)次;椎体成形术定位平均透视(4.86±1.26)次、穿刺平均透视(5.28±2.67)次。观察组定位、穿刺透视次数均少于对照组(P<0.05)。结论:胸腰椎体表定位器应用在脊柱微创手术中,可以提升定位的精准度及手术效率,减少透视、穿刺次数,降低及医患辐射量及手术操作损伤。Objective:To evaluate the effect of self-made thoracolumbar body surface locator in minimally invasive spinal surgery.Methods:139 patients with thoracolumbar diseases for radiofrequency ablation and vertebroplasty were selected from January 2017 to December 2018.They were randomly divided into observation group(27 patients with 34 segments of radiofrequency ablation,42 patients with 49 vertebral bodies of vertebroplasty)and control group(27 patients with 35 segments of radiofrequency ablation,43 patients with 45 vertebral bodies of vertebroplasty).The observation group was positioned by thoracolumbar body surface locator,while the control group was positioned by traditional Kirschner needle.The fluoroscopy and puncture times of the two groups were counted and compared.Results:In the observation group,the average number of radiofrequency ablation localization fluoroscopy was(1.86±0.94)times,the average number of puncture fluoroscopy was(4.52±2.01)times,the average number of vertebroplasty localization fluoroscopy was(2.15±1.34)times,and the average number of puncture fluoroscopy was(3.12±2.16)times.In the control group,the average fluoroscopy of radiofrequency ablation was(3.47±1.28)times,the average fluoroscopy of puncture was(8.75±3.37)times,the average fluoroscopy of vertebroplasty was(4.86±1.26)times,and the average fluoroscopy of puncture was(5.28±2.67)times.The number of localization and puncture fluoroscopy in the observation group was less than that in the control group(P<0.05).Conclusion:The application of thoracolumbar body surface locator in minimally invasive spine surgery can improve the accuracy and efficiency of localization,reduce the number of fluoroscopy and puncture,reduce the radiation dose of doctors and patients and surgical operation injury,and facilitate clinical application and promotion.
