温通膏外敷联合红外线照射治疗肌注黄体酮所致局部反应疗效及对患者免疫功能的影响

摘　　要：目的:探讨温通膏外敷联合红外线照射对黄体酮所致局部反应临床疗效及对患者免疫功能的影响。方法:选择先兆流产并肌注黄体酮病例共280例,其中出现局部反应体征病例100例,根据随机数字表法分为联合组和对照组各50例,对照组给予红外线照射治疗,联合组患者在此基础上联合温通膏外敷治疗,两组均为2周1个疗程。观察两组临床疗效,并对两组患者免疫功能水平、孕酮和β-HCG水平、症状积分变化情况进行比较。结果:联合组治疗有效率高于对照组(χ^2=8.491,P=0.014)。治疗前,两组患者免疫功能水平比较,差异无统计学意义(P>0.05),治疗后两组指标均明显上升,联合组患者CD4^+、CD8^+和CD4^+/CD8^+水平等指标上升/降低幅度更明显,差异有统计学意义(P<0.05)。治疗前,两组患者孕酮和β-HCG水平比较,差异无统计学意义(P>0.05),治疗后联合组患者孕酮和β-HCG水平等指标上升幅度更明显,差异有统计学意义(P<0.05)。治疗前,两组患者症状积分比较,差异无统计学意义(P>0.05),治疗后联合组患者小腹疼痛、阴道流血、腰酸胀痛和两膝酸软降低程度更加明显,差异有统计学意义(P<0.05)。结论:温通膏外敷联合红外线照射对黄体酮所致局部反应治疗疗效显著,可明显改善患者局部反应症状和免疫功能水平。Objective:To explore the effect of Wentong plaster combined with infrared radiation on progesterone and the immune function of patients.Methods:A total of 280 cases of threatened abortion with progesterone injection in our hospital were selected,among which 100 cases developed local reaction symptoms.According to the random number table method,the 100 patients were divided into the combined group and the control group,with 50 cases in each group.The control group was given infrared radiation treatment,and the combined group was given Wentong plaster external application for combined treatment on the basis of treatment in the control group.Both groups were treated with one cycle of two weeks.Results:The effective rate of the combined group was higher than that of the control group(χ^2=8.491,P=0.014).Before treatment,there was no significant difference in the level of immune function between the two groups(P>0.05),which,however,increased significantly in the two groups after treatment,and there was more significant increase/decrease in the level of CD4^+,CD8^+and CD4^+/CD8^+in the combined group(P<0.05).Before treatment,there was no significant difference in progesterone andβ-hCG levels between the two groups(P>0.05).After treatment,the levels of progesterone andβ-HCG increased more significantly in the combined group,with statistical difference(P<0.05).Before treatment,there was no significant difference in symptom scores between the two groups(P>0.05).After treatment,patients in the combined group had more significant reduction in abdominal pain,vaginal bleeding,backache and tenderness of the knees(P<0.05).Conclusion:External application of wentong paste combined with infrared irradiation has a significant effect on the local response of progesterone,which can significantly improve the local response symptoms and immune function of patients.
