中药缓解5-氟尿嘧啶类化疗药物所致贫血Meta分析

摘　　要：目的:系统评价分析中药缓解5-氟尿嘧啶(5-FU)类化疗药物所致贫血的疗效,筛选可以有效预防化疗所致贫血的中药处方或制剂。方法:检索中国知网、维普数据库、万方数据库、中国生物医学文献数据库中有关使用中药治疗或预防结直肠癌患者化疗所致贫血的随机对照临床试验文献。检索时限为2001年1月至2018年12月。通过纳入及排除标准,筛选切题文献,提取文献重要信息,采用RevMan 5.3软件进行Meta分析。结果:最终纳入使用相同中药处方或制剂的10篇文献进行Meta分析,包含712名受试者,4种中药处方或制剂。Meta分析结果显示中药能缓解5-FU类化疗药物所致贫血。艾迪注射液[OR值=0.26、95%CI(0.13,0.52)、P=0.0001]、四君子汤[OR值=0.30、95%CI(0.10,0.90)、P=0.03]的缓解作用比较具有统计学意义。君子扶正汤[OR值=0.87、95%CI(0.42,1.81)、P=0.71]、升血汤[OR值=0.57、95%CI(0.27,1.20)、P=0.14]的疗效在两组间比较无统计学差异。结论:中药能有效缓解5-FU类化疗药物治疗结直肠癌时所致贫血,其中艾迪注射液和四君子汤具有最佳疗效。Objective:To systematically evaluate and analyze the efficacies of traditional Chinese medicine(TCM)in alleviating anemia caused by 5-fluorouracil(5-FU)-based chemotherapeutic drugs,and to screen the effective TCM prescriptions or preparations for preventing chemotherapy-induced anemia(CIA).Methods:Searched literatures including randomized controlled trials(RCTs)on treatment or prevention of CIA in patients with colorectal cancer(CRC)with the help of CNKI,VIP,WanFang Data,and CBM from January 2001 to December 2018.According to inclusion and exclusion criteria,literatures were screened and important information was extracted.RevMan 5.3 was used for Meta-analysis.Results:A total of 10 literatures including the same TCM prescription or preparation were included for Meta-analysis,involving 712 patients and 4 prescriptions or preparations.The results of Meta-analysis confirmed that TCM can alleviate anemia caused by 5-FU-based chemotherapeutic drugs.The results of Meta-analysis showed that the efficacies of Aidi injection[OR=0.26,95%CI(0.13,0.52),P=0.0001]and Sijunzi decoction[OR=0.30,95%CI(0.10,0.90),P=0.03]were statistically significant.However,for the efficacies of Junzi Fuzheng decoction[OR=0.87,95%CI(0.42,1.81),P=0.71]and Shengxue decoction[OR=0.57,95%CI(0.27,1.20),P=0.14],there was no statistical difference between the normal control group and the experimental group.Conclusion:TCM can effectively alleviate anemia caused by 5-FU-based chemotherapeutic drugs.Aidi injection and Sijunzi decoction have the best efficacies among all reviewed prescriptions or preparations in this study.
