摘　　要：目的:探讨注射用醋酸亮丙瑞林缓释微球联合屈螺酮炔雌醇片长期维持治疗减缓Ⅲ—Ⅳ期卵巢子宫内膜异位症腹腔镜保守手术后复发的临床疗效,为临床治疗提供依据。方法:选取2016年1月至2018年1月在北京同仁医院妇科行腹腔镜下卵巢子宫内膜异位症保守手术、术中子宫内膜异位症分期(r-AFS,1996年美国生育学会修订)为Ⅲ—Ⅳ期且术后经病理确诊为卵巢子宫内膜异位症、术后门诊按时随访的76例患者为研究对象,采用抽签法将患者随机分为观察组(术后给予注射用醋酸亮丙瑞林缓释微球联合屈螺酮炔雌醇片长期维持治疗)和对照组(术后给予注射用醋酸亮丙瑞林缓释微球),每组38例。对两组患者术前和术后24个月的总主观症状评分进行比较,并比较两组患者复发时间和复发率。结果:观察组患者治疗后的总主观症状评分为(0.94±0.59)分,明显低于对照组的(2.13±0.67)分,差异有统计学意义(P<0.05)。观察组仅有1例患者复发,复发时间为术后34个月,累计复发率为2.6%;对照组5例患者复发,平均复发时间为术后18个月,累计复发率为13.1%;两组患者术后复发率的差异有统计学意义(P<0.05)。结论:Ⅲ—Ⅳ期卵巢子宫内膜异位症腹腔镜保守手术后,应用屈螺酮炔雌醇片长期维持治疗可明显缓解患者临床症状,有效降低远期复发率,延缓疾病复发时间,有一定的临床应用价值。OBJECTIVE:To probe into the clinical efficacy of leuprorelin acetate microspheres sustained release for injection combined with drospirenone and ethinylestradiol tablets long-term sequential therapy in improving recurrence of stageⅢ-Ⅳovarian endometriosis after laparoscopic conservative surgery,so as to provide evidence for clinical treatment.METHODS:Totally 76 patients with ovarian endometriosis treated by laparoscopic conservative surgery in Obstetrics and Gynecology department of Beijing Tongren Hospital from Jan.2016 to Jan.2018 were extracted.According to the 1996 r-AFS criteria,all the patients were classified as stageⅢ-Ⅳendometriosis.And the postoperative outpatient department was followed up regularly.All patients were randomly divided into the study group(received leuprorelin acetate microspheres sustained release for injection after surgery and continued with long-term sequential drospirenone and ethinylestradiol tablets therapy)and the control group(received leuprorelin acetate microspheres sustained release for injection after surgery)by lottery,with 38 cases in each group.The total subjective symptom score of two groups was calculated before surgery and after surgery of 24 months.The recurrence time and recurrence rate of two groups were compared.RESULTS:The subjective symptom score of observation groups was(0.94±0.59)scores,significantly lower than that of the control group(2.13±0.67)scores,the difference was statistically significant(P<0.05).In the observation group,there was only one patient with recurrence,the recurrence time was 34 months after the surgery,and the cumulative recurrence rate was 2.6%.In the control group,5 patients had recurrence,the average recurrence time was 18 months after surgery,and the cumulative recurrence rate was 13.1%.The postoperative recurrence rate of two groups was statistically significant(P<0.05).CONCLUSIONS:Ⅲ-Ⅳperiod ovary endometriosis after laparoscopic conservative surgery,drospirenone and ethinylestradiol tablets long-term sequential therapy ca
