117例贝伐珠单抗注射液致药品不良反应的特点分析

摘　　要：目的:探讨贝伐珠单抗注射液致药品不良反应(adverse drug reaction,ADR)的规律和特点,为合理用药提供依据。方法:收集2016年9月至2019年12月解放军总医院第四医学中心使用贝伐珠单抗注射液的病例,共纳入符合入选标准的患者117例,其中男性患者65例,女性患者52例,调取其入院记录,归纳分析发生ADR患者的原患疾病、性别、年龄以及ADR严重程度分级。结果:117例使用贝伐珠单抗注射液的患者中,发生贝伐珠单抗注射液药品说明书中提及的ADR共75例,排除合并用药因素,与贝伐珠单抗注射液高度相关的ADR有39例。贝伐珠单抗注射液主要的ADR分别为高血压(18例,发生率为15.38%)、胃肠道不适(15例,发生率为12.82%)、过敏(7例,发生率为5.98%)、出血(6例,发生率为5.13%)和蛋白尿(2例,发生率为1.71%),未发生致死性ADR。结论:贝伐珠单抗注射液所致ADR较轻微,但临床应用时应熟悉其致ADR的特点,排除使用该药可能发生严重ADR的高危人群,并对其常见的ADR进行密切监测,做到及时、有效处理。OBJECTIVE:To investigate the laws and characteristics of adverse drug reactions(ADR)induced by bevacizumab injection,and to provide evidence for rational drug use.METHODS:Patients treated with bevacizumab were collected from the Fourth Medical Center of PLA General Hospital from Sept.2016 to Dec.2019.A total of 117 patients who met the inclusion criteria were enrolled,including 65 male patients and 52 female patients.The admission records were collected to summarize and analyze the primary disease,gender,age and severity grade of ADR.RESULTS:Of the 117 patients received bevacizumab injection,75 cases had ADR mentioned in the instructions for bevacizumab injection,and 39 cases had ADR that was highly correlated with bevacizumab injection,excluding the combination of drug factors.The main ADR of bevacizumab injection were hypertension(18 cases,the incidence rate was 15.38%),gastrointestinal discomfort(15 cases,the incidence rate was 12.82%),allergy(7 cases,the incidence rate was 5.98%),hemorrhage(6 cases,the incidence rate was 5.13%)and proteinuria(2 cases,the incidence rate was 1.71%).No fatal adverse drug reactions occured.CONCLUSIONS:ADR induced by bevacizumab injection is relatively mild,yet the characteristics of ADR induced by bevacizumab injection should be understand in clinical application,so as to exclude the high-risk population that may have severe ADR induced by bevacizumab injection,and the common ADR should be closely monitored to ensure timely and effective treatment.
