解毒化痰、祛瘀通脉法治疗动脉硬化闭塞症31例的临床研究

摘　　要：目的:观察具有解毒化痰、祛瘀通脉作用的自拟方药“丹青通脉饮”治疗动脉硬化闭塞症(ASO)临床疗效。方法:选取符合纳入标准的ASO属于痰瘀阻络证患者60例为研究对象,随机分为治疗组(31例)与对照组(29例)两组。两组均予以常规西医对因治疗,治疗组口服“丹青通脉饮”,对照组口服院内制剂脉管炎片。观察治疗前后两组患者中医证候积分、踝肱指数(ABI)、血液流变学、血凝、血脂五项及同型半胱氨酸(Hcy)指标水平。结果:治疗组总有效率为90.32%(28/31),对照组总有效率为72.41%(21/29),治疗组明显高于对照组,差异有统计学意义(P<0.05);两组的相关实验室化验指标比较,治疗组的血液流变学、D-二聚体、总胆固醇(TCHOL)、甘油三酯(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)及Hcy水平均优于对照组,差异有统计学意义(P<0.05);治疗组的ABI及中医证候积分均优于对照组,差异有统计学意义(P<0.05)。结论:运用“丹青通脉饮”通过解毒化痰、祛瘀通脉法治疗ASO痰瘀阻络证能提高疗效,改善临床症状,并对血液流变学、Hcy、D-二聚体、TG、TCHOL、HDL-C及LDL-C水平干预有效。Objective:To observe the curative effect of self-made Danqing Tongmai Yin,which has the action of Jiedu Huatan Quyu Tongmai(eliminating toxin and resolving dampness,removing blood stasis and activating channels),in the treatment of arteriosclerosis obliterans(ASO).Methods:60 cases of ASO met the inclusion criteria were randomly divided into the treatment group(n=31)and the control group(n=29).Based on conventional western medication,the treatment group was treated with self-made Danqing Tongmai Yin and the control group was treated with hospital preparation of Maiguanyan Tablets.The changes of TCM symptom scores,ABI,hemorheology,hemagglutination,blood lipid and homocysteine(Hcy)were measured in the two groups.The clinical efficacy was compared between the two groups.Results:The total effective rate was 90.32%(28/31)in the treatment group,which was significantly higher than 72.41%(21/29)in the control group(P<0.05).The improvements of hemorheology,D-Dimer,total cholesterol(TCHOL),triglyceride(TG),high-density lipoprotein(HDL-C),low-density lipoprotein(LDL-C)and Hcy were more significant in the treatment group than those in the control group(P<0.05).ABI and symptom scores of the treatment group were better than those of the control group(P<0.05).Conclusion:Self-made Danqing Tongmai Yin can improve the curative effect and clinical symptoms of ASO with Jiedu Huatan Quyu Tongmai method.The therapy is effective in the intervention of hemorheology,Hcy,D-Dimer,TG,TCHOL,HDL-C and LDL-C.
