摘　　要：目的探究Prosea1喉罩对腹腔镜胆囊切除患者应激反应的影响。方法选择2017年4月至2019年2月我院择期行腹腔镜胆囊切除术的122例患者作为研究对象,按照随机数字表法将患者分为喉罩组及气管插管组,两组患者分别于术中采用喉罩及气管插管。于麻醉诱导前(T0)、置入喉罩或气管插管后1min(T1)、5min(T2)、气腹建立后20min(T3)、拔除喉罩或气管导管即刻(T4)患者平均动脉压(MAP)、心率(HR)、血氧饱和度(SaO2)、血清肾上腺素(AE)、多巴胺(DA)、皮质醇(COR)水平,记录术中及术后并发症发生情况。结果喉罩组T1、T3时间点MAP、HR指标低于气管插管组;与T0比较,T1~T4时间点AE、DA、COR指标均明显升高(P<0.05),喉罩组升高幅度低于气管插管组(P<0.05);喉罩组拔管呛咳及咽部疼痛发生率明显低于气管插管组(P<0.05)。结论Prosea1喉罩可降低腹腔镜胆囊切除患者应激反应,提高术中血流动力学稳定,降低并发症发生率。Objective To investigate the effects of Proseal laryngeal mask on stress response in patients undergoing laparoscopic cholecystectomy.Methods A total of 122 patients undergoing elective laparoscopic cholecystectomy in the hospital from April 2017 to February 2019 were enrolled.They were divided into laryngeal mask group and tracheal intubation group according to random number table method.Both groups were respectively given laryngeal mask and tracheal intubation during surgery.Before anesthesia induction(T0),at 1 min after inserting laryngeal mask or tracheal intubation(T1)and 5 min(T2),at 20 min after pneumoperitoneum establishing(T3),immediately after removing laryngeal mask or tracheal tube(T4),mean arterial pressure(MAP),heart rate(HR),blood oxygen saturation(SaO2),serum adrenaline(AE),dopamine(DA)and cortisol(COR)levels in both groups were measured.The occurrence of intraoperative and postoperative complications was recorded.Results At T1 and T3,MAP and HR in laryngeal mask group were lower than those in tracheal intubation group.Compared with those at T0,AE,DA and COR were significantly increased at T1 to T4(P<0.05).The increase amplitude in laryngeal mask group was was lower than that in tracheal intubation group(P<0.05).The incidence of extubation bucking and pharyngeal pain in laryngeal mask group was significantly lower than that in tracheal intubation group(P<0.05).Conclusion Proseal laryngeal mask can reduce stress response in patients undergoing laparoscopic cholecystectomy,improve intraoperative hemodynamic stability,and reduce incidence of complications.
