新型冠状病毒肺炎患者外周血淋巴细胞及其亚群检测的临床意义

摘　　要：目的探讨外周血淋巴细胞及其亚群在新型冠状病毒肺炎(COVID-19)患者中的应用价值。方法选取2020年2月14日~3月11日收治于广州市第八人民医院的216例COVID-19患者为研究对象,其中普通组194例,重型组15例,危重组7例,检测各组的白细胞计数、淋巴细胞计数和淋巴细胞亚群计数并进行比较。结果重型组、危重组的淋巴细胞计数、T淋巴细胞计数、CD4+T淋巴细胞计数、CD8^+T淋巴细胞计数和B淋巴细胞计数低于普通组,差异具有统计学意义(Z=-12.114^-3.376,P<0.05)。危重组的淋巴细胞计数、T淋巴细胞计数、CD4^+T淋巴细胞计数和NK细胞计数低于重型组,差异具有统计学意义(Z=-3.986^-1.975,P<0.05)。结论对COVID-19患者外周血淋巴细胞及淋巴细胞亚群进行分析,在反映COVID-19患者机体免疫系统受损程度及评估病情方面具有一定的临床应用价值。Objective To explore the application value of peripheral blood lymphocytes and lymphocyte subsets in patients with COVID-19.Methods 216 patients with COVID-19 admitted to the Eighth People’s Hospital of Guangzhou from February 14 to March 11,2020 were selected as the research object.There were 194 cases in the ordinary pneumonia group,15 cases in the severe pneumonia group and 7 cases in the critical severe group.White blood cell count,lymphocyte count and lymphocyte subgroup count of each group were detected and compared.Results The results of the lymphocyte count,T lymphocyte count,CD4^+T lymphocyte count,CD8^+T lymphocyte count and B lymphocyte count in severe pneumonia group and critical severe group were lower than ordinary pneumonia group,the differences were statistically significant(Z=-12.114^-3.376,P<0.05).The results of the lymphocyte count,T lymphocyte count,CD4+T lymphocyte count and NK cell count in the critical severe group were lower than those in the severe pneumonia group,the differences were statistically significant(Z=-3.986^-1.975,P<0.05).Conclusion Analysis of peripheral blood lymphocytes and lymphocyte subsets of patients with COVID-19 has certain clinical application value in reflecting the degree of immune system damage and evaluating the disease condition.
