腹腔镜袖状胃切除术治疗肥胖合并2型糖尿病的效果研究

摘　　要：目的:分析腹腔镜袖状胃切除术治疗肥胖合并2型糖尿病的效果。方法:2013年11月-2019年11月收治肥胖合并2型糖尿病患者80例,随机分为两组,各40例。观察组采用腹腔镜袖状胃切除术治疗;对照组采用腹腔镜下胃旁路转流术治疗。比较两组治疗效果。结果:观察组术中出血量、手术时间、术后胃肠功能恢复时间及术后住院时间均低于对照组,差异有统计学意义(P<0.05);两组术后体质指数、腰臀比及空腹血糖水平均低于术前,差异有统计学意义(P<0.05)。观察组的临床总有效率要明显高于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜袖状胃切除术相比于腹腔镜下胃旁路转流术治疗肥胖合并2型糖尿病,可获得更好的围术期指标,与此同时可获得相同的减重及降血糖效果。Objective:To analyze the effect of sleeve gastrectomy by laparoscopeon obesity complicated with type 2 diabetes.Methods:From November 2013 to November 2019,80 obesity patients with type 2 diabetes were enrolled,they were randomly divided into two groups with 40 cases in each group.The observation group was treated with sleeve gastrectomy by laparoscope.The control group was treated with gastric bypass diversion surgery by laparoscope.The therapeutic effects between the two groups was compared.Results:The amount of bleeding loss during operation,the time of operation,the time of recovery of gastrointestinal function and the length of hospital stays after operation in the observation group were lower than the control group,the differences were statistically significant(P<0.05).The postoperative body mass index,the waist-to-hip ratio and the fasting blood glucose levels in both two groups were lower than before operation,the differences were statistically significant(P<0.05)..The total clinical effective rate of the observation group was significantly higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Sleeve gastrectomy by laparoscope can obtain better perioperative indicators than gastric bypass diversion surgery by laparoscope for obesity with type 2 diabetes,at the same time,the effect of lossing weight and blood sugar lowering are the same in both methods.
