摘　　要：目的:观察联合应用小剂量多巴胺、呋塞米、乌拉地尔治疗心衰合并肾功能不全的临床疗效。方法:2016年1月-2018年12月收治心力衰竭合并肾功能不全患者81例,接受常规治疗联合多巴胺、呋塞米、乌拉地尔泵入。比较两组心功能分级、左室射血分数(LVEF)、血清肌酐、尿酸及N端前脑钠肽(NT-proBNP)水平。结果:治疗总有效率为80.2%;治疗后血清肌酐、尿酸及NT-proBNP水平均低于治疗前,LVEF水平高于治疗前,差异均有统计学意义(P<0.05)。结论:联合应用小剂量多巴胺、持续泵入呋塞米、乌拉地尔治疗心衰合并肾功能不全,能有效改善心脏射血分数及降低NT-proBNP水平,同时降低血清肌酐及尿酸水平,临床疗效显著。Objective:To explore the clinical efficacy of joint application of low dose dopamine,furosemide and urapidil in the treatment of 81 cases of heart failure with renal insufficiency.Methods:From January 2016 to December 2018,81 patients with heart failure and renal insufficiency were selected.They were treated with conventional therapy combined with dopamine,furosemide and urapidil.We compared the levels of cardiac function,left ventricular ejection fraction(LVEF),serum creatinine,uric acid and N-terminal forebrain natriuretic peptide(NT proBNP)in the two groups.Results:The total effective rate was 80.2%.After treatment,serum creatinine,uric acid and NT proBNP were lower than before treatment,and LVEF was higher than before treatment,the difference was statistically significant(P<0.05).Conclusion:The joint application of low dose dopamine,furosemide and urapidil in the treatment of heart failure with renal insufficiency can effectively improve the heart ejection fraction and reduce the NT proBNP level,and at the same time reduce the serum creatinine and uric acid level,the clinical effect is significant.
