帕金森病患者病耻感与心理社会适应及应对方式的相关性研究

摘　　要：目的探讨帕金森病患者病耻感与心理社会适应、应对方式的现状及相关性。方法采用一般资料调查表、慢性病病耻感量表(SSCI)、帕金森病患者心理社会适应量表(PAS)和医学应对方式量表(MCMQ)对266例帕金森病患者进行问卷调查。结果帕金森病患者病耻感得分(54.3511.25)分;心理社会适应总分为(83.7915.27)分;应对方式中,面对型应对方式得分(16.284.15)分、回避型应对方式得分(17.29±2.22)分、屈服型应对方式得分(11.052.86)分。帕金森病患者病耻感与心理社会适应及面对型应对方式呈负相关关系,与回避及屈服型应对方式呈正相关关系。面对和屈服型应对方式在帕金森病患者心理社会适应对病耻感的影响中起部分中介作用。结论帕金森病患者心理社会适应不良,常采用回避和屈服型应对方式,病耻感较高;心理社会适应不良的患者能直接或间接通过面对、回避或屈服型的应对方式间接影响帕金森病患者的病耻感,可通过提高心理社会适应能力和采用积极面对的应对方式改善患者的病耻感。Objective To study the current situation and relationship of psychosocial adaptation,coping style and stigma in patients with Parkinson's disease.Methods 266 Parkinson's disease patients were investigated using convenience sampling method,and were assessed with self-designed general information questionnaire,24-item Stigma scale for chronic illness(SSCI),psychosocial adjustment scale for Parkinson's disease(PAS)and medical coping modes questionnaire(MCMQ).Results The total score of stigma for patients with Parkinson's disease was(54.35±11.25),that of psychosocial adaptation was(83.79±15.27),that of coping style of confrontation was(16.28±4.15)and that of coping style of avoidance(17.29±2.22)and that of coping styles of resignation was(11.05±2.86).Psychosocial adaptation had not only direct effect on stigma,but also indirect effect on stigma through coping styles of confrontation,avoidance and resignation.Conclusion Coping styles of avoidance and resignation are important mediator of the association between psychosocial adaptation and stigma.Enhancing psychosocial adaptation and taking coping style of confrontation can effectively reduce patients'stigma.
