痰热清注射液联合左氧氟沙星治疗社区获得性肺炎的Meta及经济学分析

摘　　要：目的:系统评价痰热清注射液联合左氧氟沙星治疗社区获得性肺炎的有效性、安全性和经济性,为临床合理用药提供循证依据。方法:检索中国知网、万方数据库、维普、PubMed、Cochrane Library、BioMed Central Journals、Embase自建库至2019年2月公开发表的痰热清注射液联合左氧氟沙星与单用左氧氟沙星对社区获得性肺炎患者临床疗效的随机对照试验的文献,运用Revman5.3和State14.0软件进行Meta和药物经济学分析。结果:共纳入17篇文献,2422名患者。Meta分析结果显示,联合使用可以提高社区获得性肺炎患者的治疗有效率,减少退热时间、肺啰音时间、住院时间,用药后其不良反应无明显差异。联合用药与单用的成本-效果分析为ΔC/ΔE=-199.33。结论:联合使用痰热清和左氧氟沙星治疗社区获得性肺炎具有临床效果和经济学的优势,但仍需要通过更多高质量的随机对照试验来验证。OBJECTIVE To systematically evaluate the efficacy,safety and economy of Tanreqing injection combined with levofloxacin in the treatment of community acquired pneumonia,and to provide evidence-based basis for clinical rational drug use.METHODS The literatures of randomized controlled trials of Tanreqing injection combined with levofloxacin and levofloxacin on the clinical efficacy of patients with community-acquired pneumonia published from February 2019 to February 2019 in China Zhiwen,Wanfang database,VIP,PubMed,Cochrane Library,BioMed Central Journals and Embase were searched.Meta-analysis and pharmacoeconomic analysis were performed using Revman 5.3 and state14.0 software.RESULTS A total of 17 articles and 2422 patients were included.Meta analysis showed that combined use could improve the treatment efficiency of patients with community acquired pneumonia,reduce the time of fever abatement,lung rale and hospital stay,and there was no significant difference in adverse reactions after treatment.Cost-effectiveness analysis of combined drug use and single drug use wasΔC/ΔE=-199.33.CONCLUSION The combination of Tanreqing and levofloxacin has clinical and economic advantages in the treatment of community acquired pneumonia,but more high-quality randomized controlled trials are needed to verify it.
