吗替麦考酚酯联合激素治疗难治性过敏性紫癜疗效观察

摘　　要：目的:分析云南省第一人民医院近3年诊断为难治性腹型过敏性紫癜的患儿,观察治疗疗效,为临床上治疗儿童难治性过敏性紫癜提供方法,从而提高本病治疗疗效,减少患儿痛苦,缓解临床症状,缩短住院时间。方法:回顾性分析该院近3年初次诊断为难治性过敏性紫癜患儿63例为研究对象,根据不同的治疗方法分组,加用吗替麦考酚酯片为治疗组,使用人免疫球蛋白者对照为组1,激素改为冲击治疗量为对照组2,观察3组间消化道症状缓解时间、激素的停用时间、住院天数、不良反应及并发紫癜性肾炎的例数,进行对比。结果:在消化道症状缓解时间对照组2与治疗组、对照组1比较有显著性差异,对照组2所用时间最短,治疗组与对照组1比较有显著性差异,治疗组所用时间短,激素的停用时间治疗组与对照组1、2比较有显著性差异,住院时间3组比较有显著性差异,治疗组住院时间短于对照组1、2,不良反应及并发紫癜性肾炎3组比较无显著性差异。结论:加用吗替麦考酚酯片治疗难治性腹型过敏性紫癜有较好效果,可以较快地缓解消化道症状,又能尽早地停用激素,缩短了住院时间,且无明显不良反应发生,临床上可以选用吗替麦考酚酯片治疗难治性腹型过敏性紫癜。OBJECTIVE To analyze the children diagnosed with refractory abdominal anaphylactoid purpura in our hospital during the past 3 years, observe the therapeutic effect, and provide a method for clinical treatment of refractory anaphylactoid purpura in children, so as to improve the therapeutic effect of this disease Children suffering, relieve clinical symptoms and shorten hospital stay.METHODS The children who were diagnosed with refractory anaphylactoid purpura in our hospital in the past 3 years were recruited as the research objects. They were grouped according to different treatment methods, and the mycophenolate mofetil tablets were used as the experimental group. The patients using human immunoglobulin were selected as control group 1, and the patients with the hormone changed to shock therapy were selected as the control group 2. The time when the gastrointestinal symptoms disappeared, the time when the hormone stopped, the length of hospital stay, side effects, and the number of cases of purpura nephritis among the three group were observed.RESULTS There were significant differences in the remission time of gastrointestinal symptoms between the control group 2 and the treatment group and the control group 1. There were significant differences in the time taken by the control group 2 and the treatment group 1. There were significant differences in the time taken by the treatment group, the time of hormone withdrawal between the control group 1 and the control group 2. There were significant differences in the length of hospital stay between the three groups. There were no significant differences in the length of hospital stay between the treatment group 1 and the control group 2. There were no significant differences in adverse reactions and concurrent purpuric nephritis among the three groups.CONCLUSION The addition of mycophenolate mofetil dispersible tablets has a good effect on the treatment of refractory abdominal allergic purpura, which can quickly relieve gastrointestinal symptoms, and can stop hormone
