柴胡桂枝汤加减治疗气滞阳郁型卒中后抑郁的疗效及对5-HT水平的影响

摘　　要：目的探讨柴胡桂枝汤加减治疗卒中后抑郁(PSD)的疗效及对5-羟色胺(5-HT)水平的影响。方法将87例PSD患者按照随机数字表法随机分为治疗组(44例)和对照组(43例),对照组用盐酸帕罗西汀治疗,治疗组在对照组基础上加用柴胡桂枝汤加减治疗,疗程为4周,采用汉密尔顿抑郁量表(HAMD),于治疗前及治疗4周末评定临床疗效,同时观察2组治疗前后5-HT水平。结果治疗4周末,2组治疗前后总有效率比较有显著性差异(P<0.05);2组治疗前后HAMD评分均有显著性差异(P<0.05),2组治疗后比较有极显著性差异(P<0.01);2组治疗前后5-HT水平比较有显著性差异(P<0.05),2组治疗后比较有显著性差异(P<0.05)。结论柴胡桂枝汤加减治疗气滞阳郁型卒中后抑郁疗效好,能明显提升5-HT水平。Objective To explore the effect of modified Chaihu Guizhi decoction in the treatment of post-stroke depression of qi and yang stagnation and influence on the level of 5-HT. Methods 87 cases of post-stroke depression of qi and yang stagnation type were randomly divided into treatment group(44 cases) and control group(43 cases) according to the stochastic indicator method. The control group was treated with Paroxetine hydrochloride. The treatment group was treated with modified Chaihu Guizhi decoction and Paroxetine hydrochloride for 4 w. The effect by Hamilton Depression Rating Scale(HAMD) before treatment and the 4 th week of the treatment was evaluated, and the level of 5-HT before and after treatment was observed. Results At the end of 4 th week, the Total effective rate between two groups has significant difference(P<0.05). There were significant difference before treatment than after treatment in HAMD(P<0.05). Two groups has great significant difference(P<0.01). There were significant difference before treatment than after treatment in The level of 5-HT for two groups, and there were significant difference between two groups after treatment(P<0.05). Conclusion Modified Chaihu Guizhi decoction in the treatment of post-stroke depression of qi and yang stagnation has superiority in effect. It can significantly improve of the level of 5-HT.
