摘 要:目的探讨急性缺血性脑卒中(AIS)患者血清血管内皮钙黏蛋白(VE-cad)水平与神经功能缺损的相关性。方法 ELISA法检测161例AIS患者(AIS组)及110例其他患者(对照组)起病第1、3、10 d的血清VE-cad水平。根据NIHSS评分将患者分为轻度、中度及重度缺损亚组。结果与对照组比较,AIS组各时间点血清VE-cad的水平均显著增高(均P<0.01)。与轻度缺损亚组比较,中度和重度缺损亚组各时间点血清VE-cad水平显著增高(均P<0.05)。与中度缺损组比较,重度缺损组血清VE-cad水平显著增高(P<0.05)。AIS患者发病第1 d、第3 d、第10 d的血清VE-cad水平与NIHSS评分均呈正相关(r=0.884,r=0.809,r=0.717;均P<0.05)。轻度、中度和重度缺损亚组神经功能改善患者第3 d及第10 d VE-cad水平均显著低于第1 d(P<0.05)。结论 VE-cad参与了AIS的发生发展过程,可能是预测AIS患者神经功能缺损程度及其预后的一个重要血清标志物之一。Objective To investigate the correlation of serum vascular endothelial cadherin(VE-cad) level and the neural functional defects in patients with acute ischemia stroke(AIS).Methods The VE-cad level in 161 AIS patients(AIS group) and 110 other patients(control group) was detected by ELISA at 1, 3 and 10 d. According to the NIHSS score, patients were divided into mild defect subgroup, moderate defect subgroup and severe defect subgroup.Results Comapred with that in the control group, the level of serum VE-cad in AIS group were significantly increased at each time point(all P<0.01). Compared with that in mild defect subgroup, the levels of VE-cad in moderate defect subgroup and severe defect subgroup were significantly higher at each time point(all P<0.05). Compared with that in the moderate defect subgroup, the levels of VE-cad at each time point in severe defect subgroup were significantly higher(all P<0.05). The level of serum VE-cad at 1, 3 and 10 d was positively correlated with NIHSS score(r=0.884, r=0.809, r=0.717;allP<0.05). The levels of VE-cad in patients with improved NIHSS score in mild, moderate and severe defect subgroup at 3 and 10 d were significantly decreased compared with that on 1 d(all P<0.05).Conclusion VE-cad may be involved in the occurrence and development of AIS, and the level of VE-cad may predict the degree of neurological impairment and prognosis of AIS patients.
