摘　　要：目的了解男男性行为者(MSM)性角色类型现状,分析均可型的相关特征,为HIV干预提供依据。方法2018年2—10月采用雪球抽样,对四川省绵阳市MSM的性角色、性行为等进行问卷调查和血清学检测。运用Logistic回归模型进行性角色均可型相关特征的多因素分析。结果合格问卷回收1125份,性角色被动型35.8%,主动型25.4%,均可型38.8%。不同性角色HIV、梅毒感染率差异无统计学意义(χ^2=0.132和2.181,P均>0.05)。多因素分析结果显示,在婚/离异/丧偶(OR=6.095)、县城及其他地区居住(OR=2.284)、茶室/公园/公厕/草地寻找性伴(OR=2.469)、近6个月商业性行为(OR=6.095)、近6个月多性伴(OR=1.522)者的性角色均可型的可能大,年龄35岁以上(OR=0.386、0.446)、近1年参加社区活动(OR=0.583)者的均可型可能较小。结论当地MSM性角色以均可型占比较高,茶室/公园/公厕/草地寻找性伴大部分是均可型,更可能发生多性伴和商业性行为,此人群HIV感染风险大,应针对性加强行为干预。Objective To understand the current sexual role types of men who have sex with men(MSM),and analyze the relevant characteristics of flexible sexual behavior type,so as to provide evidences for HIV intervention.Methods From February to October 2018,snowball sampling method was used in Mianyang City,Sichuan Province to take questionnaire survey about MSM’s sexual role types and sexual behaviors and serological examination.Logistic regression model was used for multivariate analysis of flexible sexual behavior type.Results Totally 1125 valid questionnaires were collected.Passive,active and flexible types accounted for 35.8%,25.4%and 38.8%of the sexual behaviors,respectively.There was no statistical difference in infection rates of HIV and syphilis among different sexual roles(χ^2=0.132 and 2.181,P>0.05).Multivariate analysis indicated that MSM population who had the characteristics of marriage/divorce/widowhood(OR=6.095),living in county and other areas(OR=2.284),looking for a sexual partner in tea room/park/public toilet/grassland(OR=2.469),having commercial sexual behavior in the past 6 months(OR=6.095),having multiple sexual partners in the past 6 months(OR=1.522)had the great possibility of flexible sexual behavior type.MSM population who had the characteristics of age above 35 years old(OR=0.386,0.446),participating in community activities in the past 1 years(OR=0.583)had smaller possibility of flexible sexual behavior type.Conclusions Flexible sexual behavior type has a high proportion at local.MSM with this type prefer looking for a sexual partner in tea room/park/public toilet/grassland,and are more likely to have multiple partners and commercial sexual behavior,as well as high infection risk of HIV.Targeted behavior intervention should be strengthened.
