摘　　要：目的了解晚期恶性肿瘤住院患者的临终关怀服务利用和需求情况。方法从不同医疗机构和养老服务机构抽取480例晚期恶性肿瘤住院患者进行问卷调查,内容主要包括患者一般情况、生命质量、服务需求和利用方面,对问卷数据进行统计分析。结果共回收有效问卷429份(89.38%)。在不同人群对家人的理解、支持和沟通的需求上,男性选择需要及很需要208例,女性选择需要及很需要201例,差异有统计学差异(χ^2=7.184,P<0.05)。疾病信息需求上,养老院及社区医院患者选择需要及很需要168例,综合医院患者选择需要及很需要151例,差异有统计学意义(χ^2=7.384,P<0.05)。服务需求上,养老院及社区医院选择需要及很需要179例;综合医院患者选择需要及很需要169例,不同机构患者对临终关怀服务的需求有统计学差异(χ^2=17.219,P<0.05)。结论在恶性肿瘤患者中,女性对家人的理解支持和沟通的需要程度高于男性。综合医院的恶性肿瘤患者对所患疾病信息的需求和临终关怀服务的需求高于养老院及社区医院的恶性肿瘤患者。Objective To investigate the utilization and demand of hospice care services for inpatients with advanced malignant tumors.Methods A questionnaire survey was conducted among 480 inpatients with advanced malignant tumor in different medical institutions and pension service institutions,including general condition,quality of life,service demand and utilization.Then the collected data was statistically analyzed.Results A total of 429(89.38%)valid questionnaires were collected.In terms of the needs for understanding,support and communication,there was a statistically significant difference between males(208 persons needed and very needed)and females(201 persons needed and very needed)(χ^2=7.184,P<0.05).In terms of the needs for disease information,there was a statistically significant difference between patients in nursing home and community hospital(168 persons needed and very needed)and patients in general hospital(151 persons needed and very needed)(χ^2=7.384,P<0.05).In terms of the needs for hospice care,there was a significant difference between patients in nursing home and community hospital(179 persons needed and very needed)and patients in general hospital(169 persons needed and very needed)(χ^2=17.219,P<0.05).Conclusions Among patients with malignant tumors,women have a higher need for understanding,support and communication with their families than men.The demand of patients in general hospitals for information about the diseases and hospice care services is higher than that in nursing homes and community hospitals.
