摘　　要：目的观察老年人阻塞性睡眠呼吸暂停低通气综合征(OSAHS)与常见心脑血管病昼夜发作情况。方法196例老年人OSAHS(OSAHS组)同时合并高血压病、冠心病,观察1年的昼夜常见心脑血管病发作例数和次数,并与对照组比较。结果OSHAS组昼间和夜间比较(%,次数):短暂脑缺血发作(4.6,15)和(6.6,21)、缺血性脑卒中(7.1,21)和(14.8,36)、出血性脑卒中(2.6,5)和(3.1,6)、心绞痛(14.3,59)和(21.9,94)、急性心力衰竭(10.7,48)和(17.9,89)、急性心肌梗死(1.5,3)和(3.1,6)、阵发性心房颤动(6.1,15)和(8.2,34),除出血性脑卒中P>0.05,其它项均P<0.05。对照组是昼间多于夜间(P<0.05)。结论老年人OSAHS患者常见心脑血管病发作夜间较昼间显著增多,可能与夜间睡眠呼吸暂停低通气促发有关,应加强对OSAHS夜间护理观察和治疗。Objective To study the correlation between obstructive sleep apnea hypopnea syndrome(OSAHS)and the day and night attack of cardiovascular and cerebrovascular diseases in the elderly people.Methods 196 elderly OSAHS people,complicated with hypertension and coronary disease,were selected as OSAHS group.The day and night attack of cardiovascular and cerebrovascular diseases were observed in one year in OSAHS group and Control group.Results In OSAHS group,comparison between day and night attack(%,times):transient ischemic attack(4.6,15)and(6.6,21),ischemic stroke(7.1,21)and(14.8,36),hemorrhagic stroke(2.6,5)and(3.1,6),angina pectoris(14.3,59)and(21.9,94),acute heart failure(10.7,48)and(17.9,89),acute myocardial infarction(1.5,3)and(3.1,6),paroxysmal atrial fibrillation(6.1,15)and(8.2,34)were significantly different(P<0.05),except for hemorrhagic apoplexy(P>0.05).In the control group the times of day attack were more than the times of night attack(P<0.05).Conclusion OSAHS attack of elderly people is increased significantly possibly due to nocturnal sleep apnea and hypoventilation,suggesting the need to enhancing night care observation and treatment for their OSAHS.
