海藻玉壶汤联合跨学科护理模式对甲状腺肿瘤术后患者免疫功能及TSH、FT3、FT4水平的影响

摘　　要：目的观察并评价海藻玉壶汤加减联合跨学科护理模式对甲状腺肿瘤术后患者免疫功能及甲状腺激素水平的影响.方法选择2018年6月—2019年6月就诊的80例甲状腺肿瘤切除术后患者,随机分为观察组(40例)和对照组(40例).对照组口服左甲状腺素钠片50μg,终生服药;观察组在此基础上加用海藻玉壶汤加减治疗,治疗6个月;2组均给予跨学科护理模式.比较2组治疗前后甲状腺激素水平、T淋巴细胞亚群水平、中医证候积分及治疗后疗效.结果治疗后,2组各项积分均低于治疗前,观察组低于对照组(P<0.05);治疗后,2组FT3、FT4水平高于治疗前,观察组高于对照组;TSH水平低于治疗前,观察组低于对照组(P<0.05);治疗后,2组CD3^+、CD4^+、CD4^+/CD8^+细胞比例均升高,观察组高于对照组,2组CD8^+均降低,观察组低于对照组(P<0.05);观察组总有效率为85.00%,高于对照组总有效率55.00%(P<0.05).结论海藻玉壶汤加减联合跨学科护理模式可提高甲状腺肿瘤术后患者T淋巴细胞亚群水平,改善其甲状腺激素水平及临床症状,疗效显著.Objective To observe and evaluate the effect of Haizao Yuhu decoction combined with interdisciplinary nursing mode on immune function and thyroid hormone levels in patients with thyroid tumor after surgery.Methods A total of 80 patients who underwent thyroid tumor resection in our hospital between June 2018 and June 2019 were randomly divided into the observation group(40 cases)and the control group(40 cases).The control group took 50μg of levothyroxine sodium tablets orally and took them for life.The observation group added Haizao Yuhu decoction for 6 months.The two groups were given interdisciplinary nursing mode.The thyroid hormone levels,T-lymphocyte subsets,TCM syndrome scores,and curative effects of the two groups were compared before and after treatment.Results After treatment,the every scores of the two groups were lower than before,the comparison between two groups was significant(P<0.05).After treatment,both groups were higher than before with the levels of FT3 and FT4,and the comparison between two groups is significant,both groups were lower than before with the level of TSH,and the comparison between two groups was significant(P<0.05).After treatment,the CD3+,CD4+and CD4+/CD8+cells in the two groups were all increased,the observation group was higher than the control group,CD8+were decreased,the comparison between two groups was significant(P<0.05).The total effective rate in the observation group was 85.00%,and the total effective rate in the control group was 55.00%(P<0.05).Conclusion The modified Haizao Yuhu decoction combined with interdisciplinary nursing mode can increase the level of tlymphocyte subsets,improve thyroid hormone levels and TCM syndrome scores of patients with thyroid tumors after surgery,and it is a therapeutic drug worth promoting.
