激光造瘘术后庆大霉素与碘伏囊内冲洗联合加压包扎治疗耳廓假性囊肿  

Effect of Intracapsular Irrigation with Gentamycin and Iodophor Associated with Pressure Bandage after Laser Fistulas for Pseudocyst of Auricle

摘　　要：目的:探讨激光造瘘后庆大霉素与碘伏囊内冲洗联合加压包扎治疗耳廓假性囊肿的临床效果。方法:将耳廓假性囊肿患者50例随机分为观察组和对照组各25例,均经激光造瘘释放囊液后予以庆大霉素注射液和碘伏囊腔内反复冲洗,观察组注药冲洗后加压包扎,对照组不予包扎,比较两组治疗有效率、愈合时间及耳廓软骨厚度。结果:观察组治愈21例,好转4例,对照组治愈8例,好转16例,1例无效,两组有效率无显著差异(P>0.05);观察组平均愈合时间低于对照组(P<0.05);软骨增厚见于68.0%对照组患者及16%观察组患者,差异具有显著性意义(P<0.05)。两组随访12~18月均未见复发。结论:激光造瘘后囊内注射庆大霉素和碘伏治疗耳廓假性囊肿具有良好疗效,加压包扎可缩短疗程并预防软骨增厚。Objective:To evaluate the effect of intracapsular irrigation with gentamycin and iodophor associated with pressure bandage after laser fistulas for pseudocyst of auricle.Methods:50 patients with pseudocyst of auricle were divided into the observation group and the control group randomly,with 25 cases in each group.Intracapsular irrigation with gentamycin and iodophor after laser fistulas was performed in both groups,and pressure bandage was used in the observation group while not in the control group.The effective rate,healing time and thickness of auricle cartilage were compared in the two groups.Results:21 cases were cured in observation group,4 cases were improved,8 cases were cured in control group,16 cases were improved,1 case was invalid,and the two groups had no significant difference in effective rate(P>0.05);The average healing time in the observation group was lower than that in the control group(P<0.05);Thickening of auricular cartilage was more found in the control group than in the observation group(68%vs 16%),and the difference was statistically significant.Both groups had no recurrence after 12~18 months of follow-up.Conclusion:Intracapsular irrigation with gentamycin and iodophor after laser fistulas has good effect for pseudocyst of auricle,and pressure bandage will shorten the duration of symptoms and prevent auricular cartilage from thickening.
