硫酸多粘菌素B联合注射用舒巴坦钠治疗鲍曼不动杆菌感染呼吸机相关性肺炎患者的临床研究

摘　　要：目的:研究硫酸多粘菌素B联合注射用舒巴坦钠治疗鲍曼不动杆菌感染呼吸机相关性肺炎的临床效果。方法:选取2018年1月~2018年12月我院72例鲍曼不动杆菌感染呼吸机相关性肺炎患者,依照治疗方案不同分为联合用药组和对照组,各36例。两组均接受常规治疗,对照组采用注射用舒巴坦钠治疗,联合用药组采用硫酸多粘菌素B联合注射用舒巴坦钠治疗。比较两组治愈率、支气管肺泡灌洗液细菌转阴率、治疗前和治疗后临床肺部感染评分(CPIS)、血清降钙素原(PCT)水平。结果:联合用药组治愈率(77.78%)和支气管肺泡灌洗液细菌转阴率(83.33%)高于对照组(52.78%、58.33%,P<0.05)。治疗后,联合用药组CPIS评分、血清PCT水平低于对照组(P<0.05)。结论:硫酸多粘菌素B联合注射用舒巴坦钠治疗鲍曼不动杆菌感染呼吸机相关性肺炎,能有效提高患者治愈率,促进支气管肺泡灌洗液细菌转阴,减轻患者感染情况。Objective:To study the clinical effect of polymyxin B combined with sulbactam sodium in the treatment of Acinetobacter baumannii infectious ventilator-associated pneumonia.Methods:72 patients with Acinetobacter baumannii-infected ventilator-associated pneumonia from January 2018 to December 2018 were included in the study.According to different treatment options,patients were divided into a combined group and a control group,36 cases in each.Both groups were treated with conventional therapy.Additionally,the control group was given sulbactam sodium,and the combined group with additional polymyxin B on that basis.The bronchoalveolar lavage fluid conversion rate,clinical lung infection score(CPIS),and serum procalcitonin(PCT)before and after treatment,as well as the cure rate were compared between the two groups.Results:The cure rate and the convention rate of bronchoalveolar lavage fluid of the combined group(77.78%,83.33%,respectively)were higher than that of the control group(52.78%,58.33%,P<0.05).The CPIS score and serum PCT level of the combined group were lower than the control group after treatment(P<0.05).Conclusion:Polymyxin B combined with sulbactam sodium in the treatment of Acinetobacter baumannii infectious ventilator-associated pneumonia can effectively improve the cure rate,promote the conversion of bronchoalveolar lavage fluid,and reduce the infection in patients.
