摘　　要：目的探讨亚胺培南西司他丁0.5 g/2 h输注对重症肺炎患者的治疗效果。方法选择120例重症肺炎患者为试验对象,根据药物输注速度的不同将患者分为对照组和观察组,各60例。对照组给予亚胺培南西司他丁0.5 g/0.5 h输注治疗,观察组给予亚胺培南西司他丁0.5 g/2 h输注治疗。比较两组的咳嗽评分、咳嗽消失时间、肺部啰音消失时间、退热时间、住院时间、治疗效果、动脉血气指标及血清炎性因子水平。结果治疗后3 d,观察组的咳嗽评分低于对照组(P<0.05)。观察组的咳嗽消失时间、肺部啰音消失时间、退热时间及住院时间均短于对照组(P<0.05)。两组的治疗总有效率比较,差异无统计学意义(P>0.05)。治疗后,两组PaCO2均降低,PaO2、a/APO2均升高,且观察组优于对照组(P<0.05)。治疗后,观察组CRP、IL-8、IL-6水平均低于对照组(P<0.05)。结论0.5 g/2 h输注亚胺培南西司他丁治疗重症肺炎患者的效果显著,可缩短各项病症消退时间,提高治疗总有效率。Objective To explore the treatment effect of imipenem cilastatin 0.5 g/2 h infusion on patients with severe pneumonia.Methods A total of 120 patients with severe pneumonia were selected as the experimental objects,and the patients were divided into control group and observation group according to different infusion speed,with 60 cases in each group.The control group was treated with 0.5 g/0.5 h infusion imipenem cilastatin,while the observation group was treated with 0.5 g/2 h infusion imipenem cilastatin.Cough score,cough disappearance time,pulmonary rales disappearing time,antifebrile time,hospital stay time,treatment effect,arterial blood gas indexes and serum inflammatory factors levels were compared between the two groups.Results At 3 d after treatment,the cough score of the observation group was lower than that of the control group(P<0.05).The disappearance time of cough,the disappearance time of pulmonary rales,the antifebrile time and the hospital stay in the observation group were shorter than those in the control group(P<0.05).There was no significant difference in the total effective rate of treatment between the two groups(P>0.05).After treatment,PaCO2 in both groups decreased,PaO2 and a/APO2 increased,and those in the observation group were better than the control group(P<0.05).After treatment,the levels of CRP,IL-8 and IL-6 in the observation group were lower than those in the control group(P<0.05).Conclusion Imipenem cilastatin 0.5 g/2 h infusion has a significant effect in the treatment of severe pneumonia patients,which can shorten the disappearance time of symptoms and improve the total effective rate of treatment.
