腹腔镜改良膀胱颈Y-V成形术治疗前列腺切除术后膀胱颈挛缩的临床效果

摘　　要：目的探讨腹腔镜改良膀胱颈Y-V成形术治疗前列腺切除术后膀胱颈挛缩(BNC)的临床效果。方法采用随机数字表法将60例前列腺切除术后BNC患者分为对照组和观察组,每组30例。对照组给予常规经尿道手术治疗,观察组给予腹腔镜改良膀胱颈Y-V成形术治疗。比较两组的治疗效果。结果观察组手术时间及住院时间短于对照组,术中失血量少于对照组(P<0.05)。术后,观察组的RUV、Qmax、IPSS评分优于对照组(P<0.05);观察组术后12个月复发率低于对照组(P<0.05)。结论腹腔镜改良膀胱颈Y-V成形术治疗前列腺切除术后BNC的效果显著,且安全性高。Objective To explore the clinical effect of laparoscopic modified Y-V plasty of bladder neck in the treatment of bladder neck contracture(BNC)after prostatectomy.Methods Sixty patients with BNC after prostatectomy were divided into control group and observation group by random number table method,with 30 cases in each group.The control group was treated with conventional transurethral surgery,while the observation group was treated with laparoscopic modified Y-V plasty of bladder neck.The therapeutic effects of the two groups were compared.Results The operation time and hospitalization time of the observation group were shorter than those of the control group,and the intraoperative blood loss volume was less than that of the control group(P<0.05).After operation,the RUV,Qmax and IPSS score in the observation group were better than those in the control group(P<0.05);the recurrence rate at 12 months after operation in the observation group was lower than that in the control group(P<0.05).Conclusion Laparoscopic modified Y-V plasty of bladder neck is effective and safe in the treatment of BNC after prostatectomy.
