妇科腹腔镜手术麻醉中应用插管型喉罩的效果及对患者苏醒期血流动力学的影响

摘　　要：目的探讨妇科腹腔镜手术麻醉中应用插管型喉罩的效果及对患者苏醒期血流动力学的影响。方法将接受腹腔镜手术治疗的88例妇科疾病患者按随机数字表法分为参照组和研究组,各44例。参照组应用气管插管麻醉,研究组应用插管型喉罩麻醉。比较两组麻醉效果。结果停药前,两组Pmean、PETCO2均显著高于正压通气30 min(P<0.05),但组间比较,差异不显著(P>0.05)。麻醉10 min、术后即刻,两组HR、MAP均明显高于麻醉前,但研究组HR、MAP低于参照组(P<0.05)。研究组麻药使用至自主呼吸恢复时间、术毕至呼之睁眼时间、术毕至自行咳嗽时间均短于参照组(P<0.05);研究组术后躁动、呛咳、声嘶发生率均低于参照组(P<0.05)。结论妇科腹腔镜手术麻醉选用插管型喉罩可获得良好地通气效果,且对血流动力学影响较小,并发症较少。Objective To investigate the effect of intubated laryngeal mask airway in gynecological laparoscopy anesthesia and its influence on hemodynamics during recovery period of patients.Methods Eighty eight patients with gynecological diseases received laparoscopy treatment were divided into reference group and study group by random number table method,with 44 cases in each group.The reference group was anesthetized by tracheal intubation,and the study group was anesthetized by intubation laryngeal mask airway.The anesthetic effects of the two groups were compared.Results Before drug withdrawal,Pmean and PETCO2 of the two groups were significantly higher than those of positive pressure ventilation for 30 min(P<0.05),but there were no significant differences between the two groups(P>0.05).After anesthesia for 10 min and immediately after operation,HR and MAP in the two groups were significantly higher than those before anesthesia,but HR and MAP in the study group were lower than those in the reference group(P<0.05).The time from the use of anesthetics to the recovery of spontaneous breathing,the time from the end of surgery to opening eyes and the time from the end of surgery to self coughing in the study group were shorter than those in the reference group(P<0.05);the incidence of restlessness,choking cough and hoarseness after surgery in the study group were lower than those in the reference group(P<0.05).Conclusion Intubated laryngeal mask airway can achieve good ventilation effect in gynecological laparoscopy anesthesia,and have less impact on hemodynamics and fewer complications.
