弹性髓内钉内固定术与钢板内固定术治疗儿童下肢长骨骨折的效果比较

摘　　要：目的:比较弹性髓内钉内固定术与钢板内固定术治疗下肢长骨骨折患儿的效果。方法:选取80例下肢长骨骨折患儿为研究对象,按照随机数字表法分为对照组与研究组各40例。对照组行钢板内固定术治疗,研究组行弹性髓内钉内固定术治疗。比较两组临床指标水平、视觉模拟评分法(VAS)评分、髋关节Harris评分和膝关节HSS评分。结果:研究组手术时间、住院时间和骨折愈合时间均短于对照组,术中出血量少于对照组,治疗后3个月VAS评分和并发症发生率均低于对照组,Harris评分和HSS评分高于对照组,差异有统计学意义(P<0.05)。结论:弹性髓内钉内固定术治疗下肢长骨骨折患儿可缩短手术时间、住院时间和骨折愈合时间,减少术中出血量,降低治疗后3个月VAS评分和并发症发生率,以及提高Harris评分和HSS评分,效果优于钢板内固定术治疗效果。Objective:To compare effects of flexible intramedullary nail internal fixation and plate internal fixation in treatment of children with long bone fractures of lower limbs.Methods:80 children with long bone fractures of lower limbs were selected as the research objects,and were divided into control group and study group according to the random number table method,40 cases in each group.The control group was treated with plate internal fixation,while the study group was treated with flexible intramedullary nail internal fixation.Then,the level of clinical indicators,visual analogue scale(VAS)score,Harris hip score and knee HSS score were compared between the two groups.Results:The operation time,hospitalization time and fracture healing time of the study group were shorter than those of the control group.The intraoperative blood loss of the study group was less than that of the control group.The VAS score and the incidence of complications of the study group were lower than those of the control group 3 months after the treatment.The Harris score and HSS score of the study group were higher than those of the control group.All the differences were statistically significant(P<0.05).Conclusions:Flexible intramedullary nail internal fixation in the treatment of in the children with long bone fractures of lower limbs can shorten the operation time,hospital stay and fracture healing time,reduce the intraoperative blood loss,reduce the VAS score and complication rate 3 months after the treatment,and improve the Harris score And HSS score.Moreover,it is superior to plate internal fixation.
