摘　　要：目的探讨重组左旋卡尼汀注射液联合重组人促红细胞生成素(r Hu EPO)对维持性血液透析患者贫血及营养状况的影响。方法按照治疗方法不同将辽阳市中心医院2016年12月至2018年3月收治的104例维持性血液透析患者分为对照组和试验组,对照组(n=52)给予一般基础用药+r Hu EPO进行治疗,试验组(n=52)在上述治疗方案的基础上给予左旋卡尼汀注射液联合治疗。比较两组患者红细胞比容(HCT)、血红蛋白(Hb)、血清前白蛋白(PA)、血清白蛋白(ALB)、生命质量及不良反应发生情况。结果治疗后,两组患者的HCT、Hb及血清PA、ALB水平有所提高,且试验组指标显著优于对照组,差异有统计学意义(P<0.05);治疗后,两组患者F-36量表评分上升,且试验组F-36量表评分显著高于对照组,差异有统计学意义(P<0.05)。两组患者用药后不良反应发生率比较,差异无统计学意义(P>0.05)。结论左旋卡尼汀注射液与r Hu EPO联合治疗维持性血液透析患者临床效果显著,即可改善患者的营养状况,又可提高生命质量,且不良反应较少。Objective To investigate the effects of recombinant L-carnitine injection combined with recombinant human erythropoietin on anemia and nutritional status in patients with maintenance hemodialysis. Methods According to different treatment methods, a total of 104 cases of maintenance hemodialysis patients admitted to Liaoyang Central Hospital from December 2016 to March 2018 were divided into a control group and an experimental group, and the control group(n=52) was treated with general basic medication + rHuEPO, and the experimental group(n=52) was given a combination of L-carnitine injection based on the above treatment regimen. The hematocrit(HCT), hemoglobin(Hb), serum prealbumin(PA), serum albumin(ALB), quality of life and adverse reactions between the 2 groups were compared. Results After treatment, the levels of HCT, Hb, and serum PA and ALB in the 2 groups of patients increased, and the experimental group indicators were significantly better than the control group, the difference was statistically significant(P<0.05);After treatment, the F-36 scale score of the 2 groups of patients increased, and the F-36 scale score of the experimental group was significantly higher than that of the control group, the difference was statistically significant(P<0.05). There was no significant difference in the incidence of adverse reactions between the 2 groups of patients after medication(P>0.05). Conclusion The combination of L-carnitine injection and recombinant human erythropoietin in the treatment of maintenance hemodialysis patients has a significant clinical effects, which can improve the nutritional status and quality of life of patients with fewer adverse reactions.
