摘 要:目的探讨代谢指标与骨质疏松性骨折的相关性。方法选取150例骨质疏松性骨折患者为骨折组,另选取156例单纯性骨质疏松者为无骨折组,记录患者的一般资料,收集其空腹血样本用于血糖、血脂、尿酸、HbA1c等检测。结果与无骨折组相比,骨折组BMI、糖尿病比率、高脂血症比率、TC、LDL-C、FBG和HbA1c均升高,高尿酸血症比率、HDL-C和尿酸水平均降低,且差异均有统计学意义(P<0.05);Logistic回归分析提示,BMI (OR=2.214,95%CI 1.673-3.254,P=0.001)、LDL-C(OR=1.918,95%CI 1.452-2.34,P=0.021)、HDL-C(OR=0.644,95%CI 0.425-0.813,P=0.006)、HbA1c(OR=2.467,95%CI 1.997-3.892,P<0.001)和尿酸(OR=0.711,95%CI 0.478-0.892,P=0.011)是骨质疏松患者发生骨折的独立影响因素;分别根据BMI、LDL-C、HDL-C、HbA1c和尿酸水平三分位数将骨质疏松患者分成三组,结果提示随着BMI、LDL-C和HbA1c水平的升高,骨折的发生率逐渐升高;随着HDL-C和尿酸水平的升高,骨折的发生率逐渐降低。结论代谢指标如BMI、LDL-C、HDL-C、HbA1c和尿酸是骨质疏松患者发生骨折的独立影响因素。Objective To investigate the correlation between metabolic parameters and osteoporotic fracture.Methods 150 patients with osteoporotic fractures were selected as the fracture group,and 156 patients with osteoporosis were selected as the non-fracture group.Anthropometric data was collected and glucose,lipid profile,uric acid and HbA1 c were determined in all patients.Results Compared with non-fracture group,patients in the fracture group had higher BMI,diabetes rate,hyperlipidemia rate,TC,LDL-C,FBG and HbA1 c,while with lower hyperuricemia rate,HDL-C and uric acid(P<0.05).Logistic regression analysis suggested that,BMI(OR=2.214,95%CI 1.673-3.254,P=0.001),LDL-C(OR=1.918,95%CI 1.452-2.34,P=0.021),HDL-C(OR=0.644,95%CI 0.425-0.813,P=0.006),HbA1 c(OR=2.467,95%CI 1.997-3.892,P<0.001) and uric acid(OR=0.711,95%CI 0.478-0.892,P=0.011) were independent factors for fractures in patients with osteoporosis.Patients with osteoporosis were divided into three groups according to the three quantiles of BMI,LDL-C,HDL-C,HbA1 c and uric acid levels,respectively,the prevalence of fracture was increased by the elevated of BMI,LDL-C and HbA1,the prevalence of fracture was decreased by the elevated of HDL-C and uric acid.Conclusion Metabolic parameters such as BMI,LDL-C,HDL-C,HbA1 c and uric acid are independent factors for fracture in patients with osteoporosis.
