摘 要:目的探讨我院2019年1~6月碳青霉烯耐药革兰阴性杆菌(CRO)分布特点及治疗合理性,为临床合理使用抗菌药物提供依据。方法回顾性分析我院2019年1~6月CRO感染病例资料,并对结果进行统计分析。结果我院2019年1~6月共检出CRO 60株,检出率1.43%,主要为碳青霉烯耐药大肠埃希菌(CREC)5株(8.3%)、碳青霉烯耐药肺炎克雷伯菌(CRKP)11株(18.3%)、碳青霉烯耐药铜绿假单胞菌(CRPA)14株(23.4%)、碳青霉烯耐药鲍曼不动杆菌(CRAB)30株(50.0%);CRO标本来源主要为痰液(56株)、血液(2株)、中段尿液(2株)。CRO对大部分抗菌药物耐药,仅对少数氨基糖苷类、氟喹诺酮类、米诺环素、复方新诺明、替加环素(除铜绿假单胞菌)敏感。结论CRO已成为医院内感染的重要病原菌,应加强多重耐药菌的监管及碳青霉烯类抗菌药物的合理使用,注意手卫生,减少或避免多重耐药菌的产生。Objective To explore the distribution characteristics and treatment rationality of carbapenem-resistant Gram-negative bacilli(CRO)in our hospital from January to June 2019,and provide evidence for clinical rational use of antibiotics.Methods The data of CRO infection in our hospital from January to June 2019 were retrospectively analyzed,and the results were statistically analyzed.Results A total of 60 CRO strains were detected by our hospital from January to June 2019.The detection rate was 1.43%,mainly carbapenem-resistant Escherichia coli(CREC)5 strains(8.3%),carbapenem-resistant Klebsiella pneumoniae(CRKP)11 strains(18.3%),carbapenem-resistant Pseudomonas aeruginosa(CRPA)14 strains(23.4%),and carbapenem-resistant Acinetobacter baumannii(CRAB)30 strains(50.0%).The CRO specimens were mainly from sputum(56 strains),blood(2 strains),and mid-stage urine(2 strains).CRO was resistant to most antibacterial drugs,and was only sensitive to a few Aminoglycosides,Fluoroquinolones,Minocycline,Compound sinomine,Tigecycline(except Pseudomonas aeruginosa).Conclusion CRO has become an important pathogen in the hospital infection.The supervision of multi-drug resistant bacteria and the rational use of carbapenem antibacterial drugs should be strengthened,and hand hygiene should be paid attention to reduce or avoid the production of multi-drug resistant bacteria.
