摘　　要：目的研究促红细胞生成素(erythropoietin,EPO)联合输血治疗对慢性贫血患者的疗效及血液学相关指标的影响。方法选取2017年2月至2018年2月于我院血液科就诊的108例慢性贫血患者,按照随机数表法分为观察组(n=56)及对照组(n=52)。对照组接受成分输血治疗,观察组在对照组治疗基础上联合EPO治疗。观察两组疗效情况、输血情况(每次输血间隔时间、平均输血量)、Karnofsky功能状态量表评分、EPO、血清铁、总铁结合力及铁蛋白水平,血液学指标红细胞压积(hematocrit,HCT)、血红蛋白(hemoglobin,Hb)、红细胞计数(red blood cell count,RBC)及不良反应发生率。结果治疗前,两组EPO、血清铁、总铁结合力、铁蛋白水平及血液学指标(HCT、Hb、RBC)均无显著差异(P>0.05);治疗后,观察组总有效率、每次输血间隔时间、Karnofsky功能状态量表评分及血液学指标均高于对照组(P<0.05),观察组平均输血量及不良反应总发生率低于对照组(P<0.05)。结论EPO联合输血治疗对慢性贫血患者的疗效显著,可有效改善其贫血状态,恢复血液学相关指标,且不良反应少,安全性较高。Objective Study the effect of erythropoietin(EPO)combined with blood transfusion on the efficacy and hematological parameters of patients with chronic anemia.Methods A total of 108 patients were selected as study subjects who were treated from February 2017 to February 2018 in our hospital.The patients were divided into two groups by using of random number table:the observation group(n=56)and the control group(n=52).The control group was treated with blood component transfusion,while the observation group treated with EPO combined with blood component transfusion.For the two groups of patients,the therapeutic efficacy,blood transfusion(blood transfusion interval and average volume),KPS scale score of efficacy,levels of EPO,serum iron,total iron binding capacity and ferritin were measured.Hematological indicators,including hematocrit(HCT),hemoglobin(Hb),red blood cells(RBC)were also tested.And the adverse reactions were recorded.Results Before treatment,there was no significant difference in EPO,serum iron,total iron binding capacity,ferritin level and hematological indexes(HCT,Hb,RBC)between the two groups(P>0.05).After treatment,the total effective rate of observational group was higher than that of control group(P<0.05).For the patients of observation group,the interval time of each transfusion was longer than that of the control group(P<0.05),the average blood transfusion volume was significantly lower than that of the control group(P<0.05),the efficacy of KPS scale score was higher than that of control group(P<0.05),HCT,Hb and RBC were higher than those in the control group(P<0.05).The total incidence of adverse reactions was lower than that in the control group(P<0.05).Conclusion EPO combined with blood transfusion has a significant effect on patients with chronic anemia.It can effectively relieve anemia status,restore hematological parameters,with few adverse reactions and high safety.
