地佐辛在老年患者腹腔镜手术后镇痛中的应用效果分析

摘　　要：目的:探讨地佐辛在老年患者腹腔镜手术后镇痛中的应用效果。方法:选择2016年1月至2019年8月腹腔镜手术后转入重症监护室的93例老年患者,随机分为对照组(n=45)与观察组(n=48)。对照组使用舒芬太尼镇痛,观察组使用地佐辛镇痛,镇痛目标为面部表情评分0~3分;两组均予以咪达唑仑镇静,镇静目标为Richmond躁动-镇静量表评分-2~1分。观察两组患者治疗前及治疗后1 h、12 h、24 h、48 h呼吸频率及血氧饱和度、平均动脉压、心率、面部表情评分、Richmond躁动-镇静量表评分、药物不良反应发生情况。结果:观察组患者用药12 h后面部表情评分、Richmond躁动-镇静量表评分均低于对照组,差异有统计学意义(P<0.05)。观察组患者恶心、呕吐发生率较对照组低,差异有统计学意义(P<0.05)。结论:地佐辛应用于老年患者腹腔镜手术后镇痛具有良好的有效性及更高的安全性。Objective:To explore the application of dezocine in analgesia after laparoscopic surgery in elderly patients.Methods:From Jan.2016 to Aug.2019,93 elderly patients who were transferred to intensive care unit after laparoscopic surgery were selected and randomly divided into a control group of 45 cases and an observation group of 48 cases.The control group was given sufentanil for analgesia,while the observation group was given dezocine for analgesia,the target of analgesia was faces pain scale 0-3.Both groups were given midazolam for sedation,the sedation target was Richmond agitation sedation scale score-2~1.The respiratory rate,blood oxygen saturation,mean arterial pressure,heart rate,faces pain scale,Richmond agitation sedation scale score and adverse drug reactions were compared between two groups before treatment and 1 h,12 h,24 h,48 h after treatment.Results:The scores of faces pain scale and the Richmond agitation sedation scale of the observation group were significantly lower than those of the control group after 12 h of medication,the difference was statistically significant(P<0.05).The incidence of nausea and vomiting in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusions:In elderly patients with laparoscopic postoperative analgesia,dezocine has good effectiveness and higher safety.
