摘 要:目的探讨关节镜下肘关节松解术后行持续被动训练(CPM)同时联合局部冰敷的效果。方法2018年1月~2020年1月我院收治15例行肘关节镜辅助治疗肘关节僵硬、粘连等肘部功能障碍患者,术后第二日起采用CPM康复治疗的同时运用冰袋行局部冰敷。结果15例患者术后均无并发症,顺利康复;术后24h、48h、72h、4d、7d患肢肿胀度及疼痛评分比较,差异有统计学意义(P<0.001);术后1、2、3周肘关节主动屈曲活动度比较,差异有统计学意义(P<0.001),肘关节伸直均达到0°,Mayo肘关节功能评定比较,术后1周优良率为33.33%,术后2周优良率为46.67%,术后3周优良率为60%。结论通过术后快速CPM康复治疗同步联合局部冰敷,有效减轻康复锻炼时活动性疼痛,提高患者锻炼依从性,减少出血,早期消肿,节约单独冷敷时间以增加CPM频次,改善肘关节功能,在促进肘关节功能早期康复方面具有显著疗效。Objective To explore the effect of continuous passive motion(CPM)combined with local ice compress after arthroscopic elbow joint release.Methods From January 2018 to January 2020,15 patients with elbow dysfunctions such as elbow joint stiffness and adhesion were treated with supplementary elbow arthroscopy in our hospital.From the second day after operation on,in addition to CPM rehabilitation therapy,ice packs were used for local ice compress.Results All the 15 patients as research subjects had no postoperative complications and recovered smoothly.The swelling degrees and pain scores of the affected limb were compared among 24h,48h,72h,4d and 7d after operation,with P<0.001.The active flexion activity degrees of elbow joint were compared among 1w,2w and 3w after operation,with P<0.001,and elbow joint extension each time reached 0°.Evaluation by MEPS showed the superior rate of 33.33%at 1w after operation,46.67%at 2w after operation,and 60%at 3w after operation.Conclusion The rapid postoperative CPM rehabilitation therapy combined with local ice compress can effectively reduce the active pain during rehabilitation exercise,improve the exercise compliance of patients,reduce bleeding,disperse a swelling in the early stage,save solely cold compress time to increase CPM frequency,and improve elbow joint function.It has significant efficacy for promoting the early rehabilitation of elbow joint function.
