输液泵在小儿体外纠正PICC置管异位颈内静脉中的应用研究

摘　　要：目的:探讨输液泵对小儿体外纠正经外周静脉置入中心静脉导管(peripherally inserted central catheter,PICC)置管异位颈内静脉中的应用价值.方法:选取2018年7月-2019年11月本院收治的PICC置管异位颈内静脉患儿60例的临床资料进行回顾性分析.按导管复位方法不同分为对照组(常规重新送管法复位)与试验组(体外导管连接输液泵输液法复位),每组30例.比较两组临床指标、导管复位成功率、复位前后疼痛程度、焦虑及并发症发生情况.结果:试验组复位过程出血量、平均复位时间及平均花费均低于对照组(P<0.05).复位后,试验组Wong-Baker面部表情疼痛量表评分与改良耶鲁围术期焦虑量表(the modified Yale preoperative anxiety scale,m-YPAS)评分均低于对照组(P<0.05).试验组导管复位成功率为96.67%高于对照组的73.33%(P<0.05).试验组并发症发生率为3.33%低于对照组的23.33%(P<0.05).结论:体外导管连接输液泵输液法可有效提高PICC置管异位颈内静脉患儿复位成功率,缓解患儿疼痛及焦虑情况,安全性较好.Objective:To explore the application value of infusion pump in children vitro correction of peripherally inserted central catheter(PICC)catheterization ectopic internal jugular.Method:The clinical data of 60 children with PICC catheterization and ectopic internal jugular admitted to our hospital from July 2018 to November 2019 were retrospectively analyzed.According to different catheter reduction methods,they were divided into control group(conventional redelivery method)and experimental group(external catheter connection infusion pump infusion method),with 30 cases in each group.Clinical indicators,success rate of catheter reduction,pain degree,anxiety before and after reduction and complications were compared between the two groups.Result:The amount of bleeding during reduction,the average time of reduction and the average cost in the experimental group were lower than those in the control group(P<0.05).After reduction,the scores of Wong-Baker facial expression pain scale and the modified Yale perioperative anxiety scale(m-YPAS)in the experimental group were lower than those in the control group(P<0.05).The success rate of catheter reduction in the experimental group was 96.67%,higher than 73.33%in the control group(P<0.05).The incidence of complications in the experimental group was 3.33%,lower than 23.33%in the control group(P<0.05).Conclusion:The method of external catheter connected with infusion pump can effectively improve the catheter reduction success rate of PICC ectopic internal jugular in children,relieve children’s pain and anxiety,and has good safety.
