摘  要:目的:探讨猪肺磷脂联合经鼻双水平正压通气(BiPAP)及持续气道正压通气(NCPAP)对新生儿呼吸窘迫综合征(NRDS)患儿血气指标及氧合指数(OI)的影响。方法:选取2017年12月-2019年11月本院治疗的80例NRDS患儿的临床资料进行回顾性分析,依据无创通气方式不同将其分为BiPAP组(n=42)与NCPAP组(n=38)。BiPAP组进行猪肺磷脂联合BiPAP治疗,NCPAP组进行猪肺磷脂联合NCPAP治疗。比较两组治疗前与治疗24 h后氧分压(PaO2)、二氧化碳分压(PaCO2)、OI、无创通气时间、全肠道喂养时间、转为有创机械通气率及并发症发生情况。结果:治疗24 h后,BiPAP组OI高于NCPAP组(P<0.05)。BiPAP组无创通气时间短于NCPAP组(P<0.05)。BiPAP组转为有创机械通气率与支气管肺发育不良发生率均低于NCPAP组(P<0.05)。两组治疗24 h后PaO2、PaCO2、全肠道喂养时间、腹胀与气胸发生率比较,差异均无统计学意义(P>0.05)。结论:猪肺磷脂联合BiPAP可有效缩短NRDS患儿无创通气时间,利于改善OI,降低肺组织发育不良及转为有创机械通气发生率。Objective:To investigate the effect of porcine pulmonary phospholipid combined with biphasic positive airway pressure(BiPAP)and continuous positive airway pressure(NCPAP)on blood gas index and oxygenation index(OI)in children with neonatal respiratory distress syndrome.Method:The clinical data of 80 children with NRDS treated in our hospital from December 2017 to November 2019 were retrospectively analyzed.They were divided into BiPAP group(n=42)and NCPAP group(n=38)according to the different modes of noninvasive ventilation.The BiPAP group was treated with porcine pulmonary phospholipid combined with BiPAP,and the NCPAP group was treated with porcine pulmonary phospholipid combined with NCPAP.The oxygen partial pressure(PaO2),carbon dioxide partial pressure(PaCO2),OI before and after 24 h of treatment,the duration of non-invasive ventilation,total intestinal feeding time,conversion rate of invasive mechanical ventilation and complications were compared between the two groups.Result:After 24 h of treatment,the OI of the BiPAP group was higher than that of the NCPAP group(P<0.05).The duration of non-invasive ventilation in the BiPAP group was lower than that in the NCPAP group(P<0.05).The conversion rate of invasive mechanical ventilation and the incidence of bronchopulmonary dysplasia in the BiPAP group were lower than those in the NCPAP group(P<0.05).There were no statistically significant differences between the two groups in PaO2,PaCO2 after 24 h of treatment,total intestinal feeding time,abdominal distension and pneumothorax incidence(P>0.05).Conclusion:Porcine pulmonary phospholipid combined with BiPAP can effectively shorten the non-invasive ventilation time in children with NRDS,and it is beneficial to improve blood gas index and OI,reduce the incidence of pulmonary tissue dysplasia and conversion to invasive mechanical ventilation.
