胸部影像对成人麻疹合并肺感染的诊断价值

摘　　要：目的探讨胸部影像在成人麻疹患者肺部感染中的诊断价值。方法以胸部CT为标准,统计X线胸片发现成人麻疹出现肺部病变的灵敏度和特异度。将成人麻疹患者出现肺感染患者分为成人麻疹病毒肺炎组11例和疑似成人麻疹病毒合并细菌感染组28例,分析2组的CT肺部特征,对比2组肺部CT特征发生率。结果胸片发现成人麻疹合并肺部病变灵敏度为39.4%,特异度为72.7%。成人麻疹病毒肺炎组和疑似成人麻疹病毒合并细菌感染组中,小结节影、磨玻璃影、小叶间质增厚、斑片影、条索影、叶间胸膜增厚、小结节磨玻璃共存及小结节磨玻璃小叶间质增厚共存双侧取值等时2组间差异均无统计学意义(P>0.05);但是,如果根据实际情况选择单侧检验,小结节影磨玻璃影共存差异有统计学意义(P<0.05)。结论X线胸片对成人麻疹肺感染诊断价值有一定局限性,胸部薄层CT对成人麻疹病毒肺部感染的诊断价值值得参考。Objective To investigate the clinical significance of chest imaging in diagnosis of adult measles complicated by pulmonary infection.Methods Taking the examination resuts of chest CT as standard,the sensitivity and specificity of chest X-ray in detecting lung lesions in adult patients with measles were observed.The adult patients with measles complicated by pulmonary infection were divided into two groups:adult measles virus pneumonia group(group A,n=11)and suspected adult measles virus combined with bacterial infection group(group B,n=28).The lung characteristics by CT examination and the incidence rate of chest CT findings were observed and compared between the two groups.Results The chest X-ray found that the sensitivity and specificity in group A were 39.4%and 72.2%,respectively.There were no significant differences in nodules,ground-glass opacity,lobular interstitial thickening,patch shadows,stripe shadows,interlobar pleural thickening between the two groups(P>0.05).However,if one side test was selected according to the actual situation,there was a significant difference in the coexistence of small nodules with ground-glass opacities between the two groups(P<0.05).Conclusion Chest X-ray has certain limitations in the diagnosis of adult measles with pulmonary infection.The diagnostic value of chest thin slice CT in diagnosis of adult measles with pulmonary infection is worth reference.
