摘　　要：目的观察阿奇霉素联合布地奈德对支原体肺炎(MPP)患儿免疫应激指标、生活质量的影响。方法选取2013年10月至2016年6月诊治的MPP患儿110例,随机分为对照组和观察组,每组55例。2组均给予阿奇霉素治疗,观察组在对照组基础上加用布地奈德治疗。2组疗程均为2周。观察2组治疗前后Th17/Treg细胞比例及相关细胞因子[白介素-10(IL-10)、白介素-17(IL-17)]、转录因子(Foxp3、RORγt)、氧化应激指标[谷胱甘肽过氧化物酶(GSH-Px)、超氧化物歧化酶(SOD)、丙二醛(MDA)]、D-二聚体(D-D)、红细胞沉降率(ESR)、生活质量变化。结果治疗前2组Th17/Treg细胞比例、IL-10、IL-17、Foxp3、RORγt mRNA表达、GSH-Px、SOD、MDA、D-D、ESR、SF-36量表评分比较差异无统计学意义(P>0.05);治疗后2组Th17/Treg细胞比例、IL-17、RORγt mRNA表达、MDA、D-D、ESR均明显低于治疗前,IL-10、Foxp3 mRNA表达、GSH-Px、SOD、SF-36量表评分均明显高于治疗前,差异有统计学意义(P<0.05);且观察组与对照组比较差异有统计学意义(P<0.05)。结论阿奇霉素联合布地奈德治疗MPP疗效确切,能够明显改善患儿免疫应激指标和生活质量,值得临床推广应用。Objective To investigate the effects of azithromycin combined with budesonide therapy on immune stress indexes and life quality in children with mycoplasma pneumonia(MPP).Methods A total of 110 children patients with MPP who were admitted and treated in our hospital from October 2013 to Junuary 2016 were enrolled in the study,who were randomly divided into observation group(n=55)and control group(n=55).The patients in both groups were treated by azithromycin treatment,on the basis of control group,the patients in observation group were treated by budesonide,with a treatment course of two weeks for both groups.The changes of Th17/Treg cells and related cytokines(IL-10 and IL-17),transcription factors(Foxp3 and RORγt),oxidative stress(GSH-Px,SOD and MDA),D-D,ESR and life quality before and after treatment were observed and compared between the two groups.Results Before treatment there were no significant differences in the ratio of Th17/Treg cells,and the levels of IL-10,IL-17,Foxp3 and RORγt mRNA expression,GSH-Px,SOD,MDA,D-D,ESR and SF-36 scores between the two groups(P>0.05).After treatment,the ratio of Th17/Treg cells,the levels of IL-17,RORγt mRNA expression,MDA,D-D and ESR in both groups were significantly decreased,however,the levels of IL-10,Foxp3 mRNA expression,GSH-Px,SOD and SF-36 scores were significantly increased(P<0.05).Moreover the improve degree of these indexes in observation group was more significant than that in control group(P<0.05).Conclusion Azithromycin combined with budesonide in treatment of MPP in children has definite curative effects,which can significantly improve the immune stress indexes and life quality of patients,thus,it is worthy of clinical application and promotion.
