右美托咪定对颅内动脉瘤患者手术麻醉诱导中血流动力学的影响

摘　　要：目的:探讨右美托咪定在颅内动脉瘤患者手术麻醉诱导中对血流动力学指标的影响。方法:选取我院2015年7月至2018年10月接受颅内动脉瘤手术的66例患者,采用随机数字表法分为对照组和观察组(n=33)。观察组患者在麻醉诱导前静脉注射右美托咪定1μg·kg^(-1),对照组患者则静脉注射0.9%的氯化钠。记录两组患者T1、T2、T3、T4时间点的血流动力学指标变化,并采用统计学分析。结果:T3、T4时观察组收缩压、舒张压、心率显著低于对照组(P<0.05)。观察组的收缩压、舒张压、心率在波动值均明显小于对照组(P<0.05)。结论:在颅内动脉瘤患者手术麻醉诱导前注射右美托咪定能够将血流动学指标维持稳定状态,减少波动,有助于手术的顺利进行。Objective:To investigate the effects of dexmedetomidine on hemodynamic indexes in patients with intracranial aneurysm during surgical anesthesia induction.Methods:Sixty-six patients undergoing intracranial aneurysm surgery in our hospital from July 2015 to October 2018 were selected and divided into a control group and an observation group by random number table method(n=33).Patients in the observation group were intravenously injected with dexmedetomidine 1μg·kg-1 before induction of anesthesia,and patients in the control group were intravenously injected with 0.9%sodium chloride.Changes in hemodynamic parameters at time points T1,T2,T3,and T4 of the two groups of patients were recorded and statistical analysis was performed.Results:At T3 and T4,the systolic blood pressure,diastolic blood pressure,and heart rate in the observation group were significantly lower than those in the control group,and the differences between the groups were statistically significant(P<0.05).The fluctuations of systolic blood pressure,diastolic blood pressure and heart rate in the observation group were smaller than those in the control group,and the differences were statistically significant(P<0.05).Conclusion:Injecting dexmedetomidine before the induction of surgical anesthesia in patients with intracranial aneurysms can maintain a stable blood flow index,reduce fluctuations,and facilitate the smooth operation.
