摘 要:目的:探讨猪肺表面活性物质(PS)治疗新生儿呼吸窘迫综合征(NRDS)对患儿血气分析指标、机械通气参数的影响。方法:选取2018年6月至2019年12月于我院治疗的110例NRDS患儿,按随机数字表法分为两组,各55例。两组均行持续正压通气,对照组予以牛PS治疗,观察组采用猪PS治疗。比较两组血气分析、机械通气参数、通气时间及住院时间。结果:两组治疗前及治疗后血气分析相比,差异无统计学意义(P>0.05);两组治疗后动脉血压分压(PaO2)高于治疗前,二氧化碳分压(PaCO2)低于治疗前,差异有统计学意义(P<0.05);两组治疗前机械通气参数相比,差异无统计学意义(P>0.05);观察组治疗后吸气峰压(PIP)、呼气末正压(PEEP)及吸氧浓度(FiO2)水平为(1.85±0.11) kPa、(0.21±0.03) kPa、(51.32±3.68)%,低于对照组,差异有统计学意义(P<0.05);观察组通气时间、住院时间为(91.56±6.34) h、(17.22±4.36) d,短于对照组,差异有统计学意义(P<0.05)。结论:猪PS及牛PS均可改善NRDS患儿血气分析指标,但猪 PS对患儿呼吸功能改善效果更佳,利于降低机械通气参数,且利于缩短通气及住院时间,加快患儿康复。Objective:To investigate the effect of porcine pulmonary surfactant(PS)on blood gas analysis and mechanical ventilation parameters in neonatal respiratory distress syndrome(NRDS).Methods:110 children with NRDS who were treated in our hospital from June 2018 to December 2019 were randomly divided into two groups,55 in each group.Both groups were treated with continuous positive pressure ventilation,the control group was treated with bovine PS,and the observation group was treated with pig PS.Blood gas analysis,mechanical ventilation parameters,ventilation time and hospitalization time were compared between the two groups.result There was no significant difference between the two groups in blood gas analysis before and after treatment(P>0.05);PaO2 after treatment was higher than that before treatment,and PaCO2 was lower than that before treatment(P<0.05);there was no significant difference between the two groups in mechanical ventilation parameters before treatment(P>0.05);the levels of PIP,peep and FiO2 in the observation group after treatment were(1.85±0.11)kPa,(0.21±0.11)kPa The difference was statistically significant(P<0.05);the ventilation time and hospitalization time in the observation group were(91.56±6.34)h,(17.22±4.36)d,shorter than those in the control group(P<0.05).Conclusion:Pig PS and cow PS can improve the blood gas analysis index of NRDS,but pig PS has better effect on the respiratory function of NRDS,which is beneficial to reduce the parameters of mechanical ventilation,shorten the time of ventilation and hospitalization,and accelerate the recovery of NRDS.
