大黄芍药汤与腹腔镜手术治疗老年急性化脓性胆囊炎的临床效果研究

摘　　要：目的:探讨大黄芍药汤与腹腔镜手术对老年急性化脓性胆囊炎患者炎性因子水平、胃肠功能恢复状况及并发症的影响。方法:选取2018年1月至2020年于我院就诊的86例老年急性化脓性胆囊炎患者,按随机数字表法分为两组,各43例。对照组采用腹腔镜手术治疗,观察组加用大黄芍药汤治疗。比较两组炎性因子水平、胃肠功能恢复时间及并发症发生状况。结果:两组术前炎性因子水平相比,差异无统计学意义(P>0.05);观察组术后C反应蛋白(CRP)、肿瘤坏死因子-α(TNF-α)水平为(10.58±1.95)mg/L、(8.53±1.47)mmol/L,低于对照组的(13.14±2.27) mg/ L、(11.65±2.04) mmol/L,差异有统计学意义(P<0.05);观察组术后进食时间、肛门排气时间、肠鸣音恢复时间为(38.53±7.86)h、(26.54±4.21)h、(16.63±2.18)h,均短于对照组的(46.24±8.17)h、(32.47±4.39)h、(19.91±2.54)h,差异有统计学意义(P <0.05);观察组并发症发生率略低于对照组,差异无统计学意义(P>0.05)。结论:大黄芍药汤联合腹腔镜手术可缩短老年急性化脓性胆囊炎患者胃肠功能恢复时间,促进炎症反应消失,加快患者术后康复,且并发症少,安全可靠。Objective:To investigate the effect of rhubarb paeoniflorin decoction and laparoscopic surgery on inflammatory factors,gastrointestinal function recovery and complications in elderly patients with acute suppurative cholecystitis.Methods:86 elderly patients with acute suppurative cholecystitis in our hospital from January 2018 to 2020 were randomly divided into two groups,43 cases in each group.The control group was treated with laparoscopic surgery,and the observation group was treated with Dahuang Shaoyao decoction.The levels of inflammatory factors,recovery time of gastrointestinal function and complications were compared between the two groups.Results:There was no significant difference in the levels of inflammatory factors between the two groups before operation(P>0.05);the levels of CRP and TNF-αin the observation group were(10.58±1.95)mg/L,(8.53±1.47)mmol/L,which were lower than those of the control group(13.14±2.27)mg/L and(11.65±2.04)mmol/L,the difference was statistically significant(P<0.05);the postoperative eating time,anal exhaust time and bowel sounds recovery in the observation group The time was(38.53±7.86)h,(26.54±4.21)h,(16.63±2.18)h,which were shorter than(46.24±8.17)h,(32.47±4.39)h,(19.91±2.54)h in the control group,the difference was statistically significant(P<0.05);the incidence of complications in the observation group was slightly lower than that in the control group,and the difference was not statistically significant(P>0.05).Conclusion:Dahuang Shaoyao decoction combined with laparoscopic surgery can shorten the recovery time of gastrointestinal function in elderly patients with acute suppurative cholecystitis,promote the disappearance of inflammatory reaction,and accelerate the recovery of patients after operation,with less complications,safe and reliable.
