摘　　要：目的:分析北京市大兴区某三级医院Ⅱ类切口手术围术期预防性抗菌药物应用情况。方法:对北京市大兴区某三级医院2017年1月1日-12月31日4196份Ⅱ类切口手术病历围术期抗菌药物用药情况进行回顾性分析,全面探讨抗菌药物选择分布情况、预防用药疗程分布情况、联合用药情况。结果:4196例入选病历,男353例,女3843例,中位年龄33.13(14.26,68.52)岁。4063份病历预防性使用抗菌药物,Ⅱ类切口手术预防性用药占比96.83%。药物使用占比前三位品种为奥硝唑(22.72%)、头孢呋辛(20.87%)、头孢西丁(19.14%),在首剂给药时间分布方面,以“术后给药”的方式占比最高,达70.29%,用药疗程分布方面,以“>48 h”的方式占比最高,达46.28%。结论:该三级医院Ⅱ类切口手术围术期预防性使用抗菌药物存在诸多亟待解决的问题,建议改进。Objective:To analyze the application of preventive antibacterial drugs during perioperative period of typeⅡincision surgery in a tertiary hospital in Daxing District of Beijing.Method:A retrospective analysis of the perioperative antibacterial use of 4196 typeⅡincision surgery medical records from January 1 to December 31,2017 in a tertiary hospital in Daxing District,Beijing.The selection and distribution of antibacterial drugs,the distribution of preventive medication courses and the combination of drugs were comprehensively explore.Result:4196 patients were selected into the medical record,353 patients were male and 3843 patients were female,median age was 33.13(14.26,68.52)years old.Antibacterial drugs were used for preventive use in 4063 cases,and the proportion of preventive drugs for typeⅡincision surgery was 96.83%.The top three varieties with drug use were Ornidazole(22.72%),Cefuroxime(20.87%)and Cefoxitin(19.14%).In terms of the time distribution of the first dose,the composition ratio was the highest with"post-operative administration",reached for 70.29%.In terms of the distribution of medication courses,the composition ratio was the highest with">48 h",reached for 46.28%.Conclusion:There are many problems to be solved in the perioperative prophylactic use of antibiotics in typeⅡincision operation in the third level hospital,suggestions for improvement.
