固肾壮骨片联合针灸对腰椎间盘突出症患者微循环与神经递质的影响观察

摘　　要：目的:探讨固肾壮骨片联合针灸对腰椎间盘突出症患者微循环与神经递质的影响。方法:选取2017年6月-2019年11月本院90例腰椎间盘突出症患者为研究对象,根据治疗方案的不同将其分为对照组与观察组,每组45例。对照组进行针灸治疗,观察组在对照组基础上联合固肾壮骨片治疗。比较两组临床疗效、治疗前后的症状体征积分、局部血流、微循环指标及神经递质。结果:观察组治疗总有效率高于对照组(P<0.05);治疗后1、3周,观察组腰痛与下肢放射痛积分均低于对照组(P<0.05);治疗后1、3周,观察组L4、L5及S1局部血流均高于对照组(P<0.05);治疗后1、3周,观察组白微栓率、红细胞聚集率及管襻畸形率均低于对照组(P<0.05);治疗后1、3周,观察组神经肽Y(neuropeptide Y,NPY)、β-内啡肽(β-endorphin,β-EP)及多巴胺(DA)水平均低于对照组(P<0.05)。结论:固肾壮骨片联合针灸在腰椎间盘突出症患者中的治疗效果较好,可有效改善微循环与神经递质,应用价值较高。Objective:To investigate the effects of Gushen Zhuanggu Tablet combined with acupuncture on microcirculation and neurotransmitters in patients with lumbar disc herniation.Method:A total of 90 patients with lumbar disc herniation in our hospital from June 2017 to November 2019 were selected as study subjects.They were divided into control group and observation group according to different treatment plans,45 patients in each group.The control group was treated with acupuncture,and the observation group was treated with Gushen Zhuanggu Tablets on the basis of the control group.Clinical efficacy,symptom and sign scores,local blood flow,microcirculation indexes and neurotransmitters before and after treatment were compared between the two groups.Result:The total effective rate of the observation group was higher than that of the control group(P<0.05).At 1 and 3 weeks after treatment,the scores of lumbago and lower limb radiation pain in the observation group were lower than those in the control group(P<0.05).At 1 and 3 weeks after treatment,local blood flow in L4,L5 and S1 in the observation group were higher than those in the control group(P<0.05).At 1 and 3 weeks after treatment,the rate of white microthrombus,erythrocyte aggregation rate and vessel loop deformity rate in the observation group were lower than those in the control group(P<0.05).At 1 and 3 weeks after treatment,neuropeptide Y(NPY),β-endorphin(β-EP)and dopamine(DA)levels in the observation group were lower than those in the control group(P<0.05).Conclusion:Gushen Zhuanggu Tablet combined with acupuncture in the treatment of lumbar disc herniation has good therapeutic effect.It can effectively improve microcirculation and neurotransmitters,and has high application value.
