开颅夹闭术治疗急性期高分级前循环动脉瘤的效果研究

摘　　要：目的:探讨开颅夹闭术治疗急性期高分级前循环动脉瘤的临床效果。方法:选取笔者所在医院收治的急性期高分级的前循环动脉瘤患者140例作为研究对象,以手术方式的不同为依据进行分组,对照组70例,采用血管内介入治疗,观察组70例,开展开颅夹闭术治疗,对两组患者的治疗效果进行比较。结果:观察组患者预后良好率明显高于对照组(P<0.05);术后死亡率比较,观察组明显低于对照组(P<0.05);除脑积水外,两组患者其他各项并发症发生率比较差异无统计学意义(P>0.05);两组患者复发率比较差异有统计学意义(P<0.05);观察组患者住院时间明显长于对照组(P<0.05)。结论:予以急性期高分级的前循环动脉瘤患者开颅夹闭术治疗可取得较好的短期疗效,且能够降低复发率,但会延长患者住院时间。Objective:To investigate the clinical effect of craniotomy clipping on anterior circulation aneurysms with high grade in the acute stage.Method:A total of 140 patients with anterior circulation aneurysm of high grade in the acute stage admitted to our hospital were selected as the research object,and the patients were divided into two groups according to the different surgical methods.70 patients in the control group were treated with intravascular interventional therapy,and 70 patients in the observation group were treated with craniotomy clipping,and the therapeutic effects of the two groups were compared.Result:The good prognosis rate of the observation group was significantly higher than that of the control group(P<0.05).The postoperative mortality of the observation group was significantly lower than that of the control group(P<0.05).In addition to hydrocephaly,the other complications rate between two groups had no significant differences(P>0.05).The difference of recurrence rate between the two groups was statistically significant(P<0.05).The length of stay in the observation group was significantly longer than that in the control group(P<0.05).Conclusion:Craniotomy for anterior circulation aneurysm patients with acute stage and high grade can achieve better short-term efficacy,and can reduce the recurrence rate,but can prolong the hospital stay.
