复方黄柏液熏洗方联合仪器熏洗在吻合器痔上黏膜环切术后中的临床应用研究

摘　　要：目的:观察复方黄柏液熏洗方联合仪器熏洗在吻合器痔上黏膜环切术后效果。方法:将142例痔疮患者按随机数字表法分为2组,2组均行吻合器痔上黏膜环切术,对照组71例患者术后采用常规治疗及护理,观察组71例患者术后采用复方黄柏液熏洗方联合仪器熏洗护理。对比2组术后不同时间段视觉模拟评分法(VAS)评分;对比2组术后并发症、并发症消失时间及创面愈合时间。结果:观察组术后第3天、第5天、第7天的VAS评分均较对照组降低,差异有统计学意义(P<0.05)。观察组肛门坠胀、便血、瘙痒、水肿、渗液、疼痛等并发症发生率较对照组低,差异有统计学意义(P<0.05)。观察组肛门坠胀、便血、瘙痒、水肿、渗液、疼痛的消失时间以及创面愈合时间均较对照组短,差异有统计学意义(P<0.05)。结论:复方黄柏液熏洗方联合仪器熏洗用于吻合器痔上黏膜环切术后的效果较显著,可有效减轻患者疼痛,使症状消失的时间缩短,加快创面的愈合,促进患者康复。Objective:To observe the postoperative effect of fumigation and washing prescription by compound Huangbai liquid combined instrument of fumigation and washing after procedure for prolapse and hemorrhoid.Methods:A total of 142 cases of patients with hemorrhoids were divided into two groups according to the random number table method,71 cases in each group.Both groups were conducted procedure for prolapse and hemorrhoid.After surgery,the control group was given routine treatment and nursing;the observation group was given fumigation and washing prescription with compound Huangbai liquid combined with nursing through combined instrument of fumigation and washing.After surgery,the scores of Visual Analogue Scale(VAS)at different periods were compared between the two groups;the complications,the disappearance time of complications and the healing time of wound surface were compared between the two groups.Results:On the 3^rd,5^th and 7^th days after surgery,the VAS score in the observation group was lower than that in the control group,the difference being significant(P<0.05).The incidence of complications such as anal downbearing distention,hematochezia,pruritus,edema,exudate and pain in the observation group was lower than that in the control group,the difference being significant(P<0.05);the disappearance time of the six complications in the observation group and the healing time of wound surface were lower than those in the control group,the difference being significant(P<0.05).Conclusion:The application of fumigation and washing prescription with compound Huangbai liquid combined with instrument of fumigation and washing has more significant effect on patients undergoing postoperative procedure for prolapse and hemorrhoid,which can effectively alleviate their pain,shorten the disappearance time of symptoms,accelerate the healing of wound surface,and promote their recovery.
